The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Vessel encountered a crank pin bearing failure on the main engine cylinder #6 resulting in operational limitation of slow ahead and astern.

In tankers; lighting in cargo-pump rooms, except emergency lighting, shall be interlocked with ventilation such that the ventilation shall be in operation when switching on the lighting . PSCO's observed ventilation not interlocked with the lighting in the pump room.
