Vessel has a 3 foot small diameter hose attached after the oil content meter, but before discharge was used by crew to verify function. The modification was not reflected in an approved system diagram plan.

The structure, machinery and equipment...of and cargo ship...shall be subject to the surveys and inspections specified below. A minimum of two inspections of the outside of the ship's bottom during any five year period. Last entry for an inspection of the outside of the ship's bottom is dated 11JAN2016. Master was able to produce an inspection report confirming a satisfactory inspection on 07DEC2018. The corresponding endorsement for this inspection is missing from the vessel's Cargo Ship Safety Construction Certificate. 74 SOLAS (14) I/10(a)(v).

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCOs discovered that the vessel's freefall lifeboat engine did not start due to dead batteries. 74SOLAS (97) III/19.2

In the event of a power failure to any one of the steering gear power units, an audible and visual alarm shall be given on the navigation bridge. During the steering gear test, the power failure alarm did not give a visual or audible alarm on the bridge or in the steering gear room; the hydraulic level alarm sounded eratically on pump one when not being tested. 74SOLAS (04) II-1/29.5.2

Due to marine casualty in Port Everglades, vessel sustained damage to the rudder. Condition of class issued for a one time voyage to Sector Jacksonville. 74SOLAS (14) I/11(c)

The machinery system and fittings shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to hot surfaces. Number one generator exhaust lagging is missing and improper for intended use. 74SOLAS 9(7) II-1/26.1

Fire fighting systems and appliances shall be maintained ready for use. PSCOs discovered that fore-peak fire station was missing fire hose and not in accordance with fire control plan. 74SOLAS (14) II-2/14.1.1

The owner, master, or person in charge of each vessel underway shall ensure that the vessel's anchors are ready for letting go. The vessel is missing starboard anchor. 33CFR164.11

Means shall be provided whereby normal operation of propulsion machinery can be sustained. Vessel's unable to engage ahead propulsion. 74 SOLAS (04Con) II/1/26.3
