THE PURPOSE OF THIS REGULATION IS TO PREVENT THE IGNITION OF COMBUSTIBLE MATERIALS OR FLAMMABLE LIQUIDS. FOR THIS PURPOSE, THE FOLLOWING FUNCTIONAL REQUIREMENTS SHALL BE MET: MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. PSCO OBSERVED ACCUMULATED POOLS OF DIESEL OIL AROUND ALL FUEL PUMPS AND CYLINDERS ON THE MAIN ENGINE. 74SOLAS(2020) II-2/4.1.1

EVERY SHIP OF 100 GROSS TONNAGE AND ABOVE, AND EVERY SHIP WHICH IS CERTIFIED TO CARRY 15 OR MORE PERSONS, AND FIXED OR FLOATING FLATFORMS SHALL CARRY A GARBAGE MANAGEMENT PLAN WHICH THE CREW SHALL FOLLOW. PSCO OBSERVED EXCESSIVE ACCUMMLATED GARBAGE IN CRANE STORE 1,2, AND 3 PILED TO THE OVERHEAD IN THE SPACE AND NO SEPARATED IN BINS PER THE VESSEL'S GARBAGE MANAGEMENT PLAN. MARPOL ANNEX V 10.2

ELECTRICAL SERVICES ESSENTIAL FOR SAFETY WILL BE ENSURED UNDER VARIOUS EMERGENCY CONDITIONS; AND THE SAFETY OF PASSENGERS, CREW AND SHIP FROM ELECTRICAL HAZARDS WILL BE ENSURED. PSCO OBSERVED 03 EMERGENCY LIGHTS NOT IN WORKING CONDITION. ADDITIONALLY 01 LIGHT ON THE STBD BRIDGE WING WAS BROKEN AT THE FRAME. 74SOLAS(20) 11-1/40.1.2,.3

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and operational nature of the deficiencies noted below provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit report satisfactory to the Coast Guard must be submitted prior to clearance of this deficiency.

A ship when in a port of another Contracting Government is subject to control by officers duly authorized by such Government concerning operational requirements in respect of the safety of ships, when there are clear grounds for believing that the master or crew are not familiar with essential shipboard procedures relating to the safety of ships. PSCO observed lack of crew familiarity with donning and using fire-fighting gear after multiple attempts.

Fire-fighters outfits shall comply with Fire Safety Systems Code. Personal equipment shall consist of protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam. PSCO observed that the fire-fighter's outfits did not provide heat or fire protection for the neck. 74 SOLAS (20) II-2/10.10.1.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed improperly running lifeboat engine. After expanding into the lifeboat maintenance record, PSCO additionally found that lifeboat was reported inoperable on 19APR2024 but not reported repaired until 20MAY2024 despite the vessel being in active use in the intervening time frame. 74 SOLAS (20) III/20.2

Each survival craft shall be stowed fully equipped as required by this chapter and the Code. PSCO observed a vessel-fabricated needle plug in place of the hydraulic release mechanism for the free-fall lifeboat. 74 SOLAS (20) III/13.1.4

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. PSCO observed the fresh water generator was inoperable and disassembled. Fresh water generator is unable to produce an adequate amount of water for ships crew and operations on board. 74 SOLAS (20) II-1/26.1

Ships shall have on board food and water supplies which, having regard to the size of the crew and the duration and nature of the voyage, are suitable in respect to quantity, nutritive value, quality and variety. PSCO observed lack of sufficient quantity of potable water on board with 75 cubic meters in the potable water tanks and 288 liters of bottled water for the entire crew for a cross-Pacific voyage. Stairways which penetrate only a single deck shall be protected, at a minimum, at one level by at least B-0 class divisions and self-closing doors. Lifts which penetrate only a single deck shall be surrounded by A-0 class divisions with steel doors at both levels. Stairways and lift trunks which penetrate more than a single deck shall be surrounded by at least A-0 class divisions and be protected by self-closing doors at all levels. PSCO observed one of the self-closing doors on the main stairway and the self-closing door in the fire control room for the emergency escape trunk to not be self-closing. One door labeled on the fire control plan as self-closing was not fitted with a self-closing device. 74 SOLAS (20) II-2/9.2.3.4.1

Ships shall be provided with arrangements, equipment and fittings of sufficient safe working load to enable the safe conduct of all towing and mooring operations associated with the normal operation of the ship. PSCO observed improper splicing on mooring line eye causing fraying. 74 SOLAS (20) II-1/3-8.2

Means shall be provided to limit the accumulation of flammable vapors. PSCO observed multiple fuel oil soaked laggings in the engine room along with fuel oil soaked rags. 74 SOLAS (20) II-2/4.1.2

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I, so that all entries in the book appropriate to that operation are completed. Each completed operation shall be signed by the officer or officers in charge of the operations concerned and each completed page shall be signed by the master of the ship. PSCO observed completed page in Oil Record Book Part I not signed by the master of the ship. MARPOL I/17.4

Each discharge into the sea or to a reception facility, or a completed incineration, shall be promptly recorded in the Garbage Record Book and signed for on the date of the discharge or incineration by the officer in charge. Each completed page of the garbage record book shall be signed by the master of the ship. PSCO observed completed page in Garbage Record Book not signed by the master of the ship. MARPOL V/10.3.1

Not less than one half of the total number of lifebuoys shall be provided with lifebuoy self-igniting lights complying with the requirements of paragraph 2.12 of the Code. PSCO observed light not igniting on lifebuoy located on the port side main deck. 74 SOLAS (20) III/7.1.3

The ring, lines and letters shall be painted in white or yellow on a dark ground or black on a light ground. PSCO observed paint on plimsol mark scratched off from wear, causing both the hull and plimsol mark to be the same color. The crew rectified this deficiency by painting plimsol mark white.

11129 - Operational Readiness - Before the ship leaves the port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed that the free fall lifeboat did not start. Batteries are inoperable even though they are connected to charger and shore-tie cable. 74SOLAS(20)III/20.2 - 17ac

07126 - Oil accumulaton in engine room. - Means shall be provided to control leaks of flammable liquids. PSCO discovered that number 1 booster fuel oil pump is actively leaking. Number 2 booster pump is leaking as well. 74SOLAS(20)II-2//4.1.1 - 17ac

07117 - Jacketed high pressure lines - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ships or persons on board. PSCO discovered that number 4 cylinder has a leak on the high pressure hydraulic line and temp repairs were made to fix the leak. 74SOLAS(20) I/11.a - 17ac

07124 - Maintenance of Fire protection systems - Maintenance, testing and inspections shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. PSCO observed engineer testing the flame detector with a lighter. Manufacturer’s manual states “check and see if the detector operates properly by using a detector tester specified by NOHMI”. - 74SOLAS(20)II-2/14.2.2.1. - 15ac
