04103-In multi-engine installations which are supplied from the same fuel source, means of isolating the fuel supply and spill piping to individual engines shall be provided. The means of isolation shall not affect the operation of the other engines and shall be operable from a position not rendered inaccessible by a fire on any of the engines. Each quick closing fuel valve for the auxiliary engines was rendered not operational due to the valve mechanism being tied with cloth by the crew. The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Objective evidence found during an expanded ISM exam revealed that the vessel’s crew failed to follow the ship's SMS procedures with regard to fire-fighting procedures (SMS Section 10.1.1) and navigation safety (SMS Section 6). The crew failed two fire drills because they were not familiar with their duties, required fire-fighting equipment, and vessel fire-fighting procedures. Additionally, the crew failed to ensure that all ECDIS charts on board are corrected/up-to-date in accordance with SMS procedures. It is recommended that an external third party audit of the SMS be conducted with regard to deficiencies no. 3 and no.4 (ISM Code 7)

Each liferaft or group of liferafts shall be stowed with a float-free arrangement complying with the requirements or paragraph 4.1.6 of the Code so that each floats from and, if inflatable, inflates automatically when the ship sinks. The weak link for the hydrostatic release for the starboard side liferaft is disconnected due to corrosion on the liferaft cradle which compromises the approved float-free arrangement. Before the ship leaves port and at all times during the voyage all lifesaving appliances shall be in good working order and ready for immediate use. The port side embarkation ladder is missing several rubber spacers which hold individual steps in place and ensure personnel can safely embark the liferaft during an emergency abandon ship. In addition, the lines holding the rubber spacers in place on the starboard side embarkation ladder are in a severe state of degradation. (74 SOLAS (14) III/20.2)

Sanitary facilities for all members of the crew who do not occupy rooms to which private or semi private facilities are attached, shall be provided one water closet for every eight persons. Two of the three common toilets for 12 rated crew members were inoperable. Additionally, one of the three common showers for the 12 rated crew was inoperable. When asked, the C/O stated the sanitary facilities have been in this condition for at least five months. (ILO-147/PWSA 46 USC 70002)

General arrangement plans shall be permanently exhibited for the guidance of the ship’s officers, showing clearly for each deck the control stations, the various fire sections enclosed by “A” class divisions, the sections enclosed by “B” class divisions together with particulars of the fire detection and fire alarm systems, the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. The vessel’s Fire Control Plan is not up-to-date and is missing information such as the location of the helideck, proper required number and locations of SCBAs, and proper required number and locations of fire-fighter outfits.

Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. In addition it shall be provided with alarm arrangements to indicate when this level cannot be maintained. The system shall also be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 ppm. The vessel’s Oil Content Meter is inoperable. The Panamanian flag state dispensation letter provided to PSCOs onboard expires August 30, 2019 and would expire before the vessel’s next intended voyage to South Korea. (MARPOL I/14.7)
