The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the authorized ballast water management methods listed in 33 CFR 151.2025(a). Vessel’s ballast water management system is inoperable. Vessel has been granted an extraordinary circumstance to employ the ballast water management method listed in 33 CFR 151.2025(a)(3) to conduct cargo operations while in the Sector New Orleans COTP Zone. In accordance with 33 CFR 151.2040(b)(3), the vessel must discharge only that amount of ballast water operationally necessary to ensure the safety and stability of the vessel for cargo operations. 33 CFR 151.2025(a), Code 40c

Resolution: Received, reviewed and accepted class report from BUREAU VERITAS and technician report stating the deficiency was rectified. Technician survey reports 12 actions taken including updating firmware on TRO units.

here remote control of propulsion machinery from the navigation bridge is provided and the machinery spaces are intended to be manned, the speed, direction of thrust and, if applicable, the pitch of the propeller shall be fully controllable from the navigation bridge under all sailing conditions, including manoeuvring. Vessel's main propulsion engine did meet proper speed and thrust while under remote control from the navigation bridge. Class Society to attend and conduct a root cause analysis report and vessel to conduct repair to the satisfaction of class.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO discoverd oil, oil soaked sorbent material and sludge accumulation in the cascade tank, indicating an oil leak in the vessel's steam system.

An alarm system shall be provided, indicating any fault requiring attention and shall be capable of sounding an audible alarm in the main machinery control room and indicate visually each separate alarm function. PSCO discovered main machinery alarm indicating multiple failures, and was not capable of sounding alarms for OWS, steering gear and high bilge level.

Each oil-fired boiler which is intended to poerate without manual supervision shall have safety arrangements which give an alarm in case of low water level, air supply failure or flange failure. The boiler load and level controllers are displaying alarms and undefined characters on the digital screen.
