Provisions shall be made to prevent over pressure in any oil tank or any part of the fuel oil system per 33CFR155.820 and 156.170. Bunker lines must be tested to 100% MAWP annually and 150%twice in any 5 yr period. The ships records show the ship tested the HFO to 4.5 bar and DO line to 3.5 bar on 18OCT2018 using air. The test shall be hydrostatic to 5 bar per ships documents. The vessel shall not bunker with pressures in excess of 3.5 bar and 4.5 bar on the DO and HFO line respectively while in US waters.

Personnel responsible for the operation of an incinerator installed in accordance with paragraph 6.1 shall be trainied to implement the guidance provided in the manufacturers operating manual as required by paragraph 7. The incinerator operating manual shows metal shall not be burned . The incinerator was found full of hot ashes including bits of metal.

Liferaft painter shall provide a connection between the ship and liferaft and shall be arranged to ensure liferaft is not dragged under by the sinking ship. Starboard liferafts did not have painters attached. Port liferaft painters were improperly installed.
