1850 - For a Protein-based alcohol-resistant foam concentrates should be subjected to a stability test with acetone. This test that is required should be performed prior to delivery to the ship and annually thereafter. PSCO's found no foam analysis report had been conducted since foam delivery on 23DEC2015. On 26FEB2018 09 out of 21 (43%) crew members were changed out. Fire and Abandon ship drills were not held until 28FEB2018 with no reason logged for not meeting 24hrs requirement. 2530 - The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations, codes, and guidelines. The company should establish procedures to ensure that the ship is maintained in conformity with provisions of relevant rules and regulations and with any additional requirements which may be established by the company. PSCO's found that safety and security drills had not been conducted after more than 25% of crew had changed out. Also safety drills logs are not kept to show when all drills have been conducted. A foam analysis has not been conducted since it was filled on 23DEC2015.
