74SOLAS(2020ed.)II-2/REG 4.1.1 The following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed fuel soaked lagging in 02 spaces, purifier room and sludge tank. Lagging around the fuel oil main unit control box and the buffer tank #2 pipe were soaked in fuel oil. The sight galss of the sludge tank also had wet oil on its lagging. Crew conducted adequate repairs.

Resolution: PSCO observed crew replace lagging in purifier room and sludge tank.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed vessel's rescue boat engine unable to stay engaged for more than 30 seconds. Solas 74(a) 2014(cons) III/20.2 17a

Action required: 17 - Rectify deficiencies prior to departure
Resolved 4 February 2019
Resolution: Received, reviewed and accepted class report from NKK stating the deficiency was rectified. Crew carried out thorough maintenance on engine:fuel filter & carburetor cleaning, spark plugs replaced,& rescue boat was refueled w/ fresh gasoline. Test sat.
