Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made with the approval of the cognizant OCMI. A crack was identified on the port end of the outside rail of the swim step. Provide repair proposal for review and approval of attending marine inspector. Once approved, restore to the satisfaction of the attending marine inspector prior to receiving credit drydock.

"A vessel must comply with the applicable requirements for any radio...including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80." The FCC Safety Radiotelephony Certificate found expired. 46 CFR 184.502

"A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have...an audible alarm that sounds upon discharge." No audible alarm witnessed by MI. 46 CFR 181.420(b)(3)

"A vessel not required to have a power driven fire pump must have at least three 9.5 liter buckets with an attached lanyard." 01 fire bucket found to be broken with no lanyard attached. 46 CFR 181.610

"A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided." 01 child life jacket found defective. 46 CFR 180.71(b)

"At least one ring life buoy must be fitted with a lifeline...be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light." MI witnessed lifeline attached to ring buoy was not dark in color/resistant to UV light or non-kinking. 46 CFR 180.70(c)(6)

All vessels must have operable navigation lights in accordance with the Navigation Rules. All navigation lights were inoperable. Provide navigation lights that are in compliance with the Navigation Rules. 46 CFR 183.420

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked "First-Aid Kit". A first-aid kit must be easily visible and readily available to the crew. First aid kit is incomplete. Provide USCG-approved kit or a kit that complies with the list in 46 CFR 160.041. 46 CFR 184.710

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant [name withheld] buckets were 3 1/2 gallons. Drill holes to 2 1/2 gallons or provide 2 1/2 gallon buckets with lanyards. 46 CFR 181.610

Electrical equipment must be installed to protect from electrical hazards. Corrosion observed on terminations of conductors in electrical panel. 46 CFR 183.200.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Bilge alarms not labeled for identification. 46 CFR 182.530(a)

When the flexible section (of a fuel fill pipe) is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Port fuel tank grounding conductor not attached to fill fitting. 46 CFR 182.445(g)

If posting of the stability letter is impracticable, the stability letter must be kept on board in a weather tight container readily available for use by the crew and display to passengers and others on request. Vessel did not have stability letter on the vessel. 46 CFR 176.306

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Provide record of supervisors receiving 60 minutes of EAP training. 46 CFR 16.401 (b)

As an employer, you must keep the following records for the following periods of time: You must keep the following records for five years: Records of verified positive drug test results. You must keep the records of negative and canceled drug test results and alcohol test results with a concentration of less than 0.02 for one year. Provide records of positive/ negative results. 49 CFR 40.333 (a)

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Onboard portable and fixed extinguishers were overdue for required annual servicing. Provide annual servicing report from a State or local jurisdiction certified fire extinguisher servicing agency.

A certification that the ship has passed an inspection must be entered into the ship's log by the inspecting technician. A FCC Safety Radiotelephony Certificate (SRC) was not onboard. Provide a valid SRC onboard.
