11101-Lifeboats-In the case of a lifeboat arranged for the free-fall launching, at least once every three months during an abandon ship drill the crew shall board the lifeboat, properly secure themselves in their seats and commence launch procedures up to, but not including, the actual release of the lifeboat. The lifeboat shall then either be free-fall launched with only the required operating crew on board, or lowered into the water by means of the secondary means of launching with ot without the operating crew on board. PSCO observed records that crew was only placing lifeboat in water every six months. Corrective action plan must be implemented by company in concurrence with Administration. 74SOLASam 14) III/19.3.4.4 -17b

Vessel is outfitted with an approved alternate management system (AMS) described in 33 CFR 151.2025(a)(3), but failed to use the AMS while conducting ballast water exchange prior to entry to US waters. In addition, ballast water management plan failed to provide crew with information about United States ballast water requirements in accordance with 33CFR151. 07106-Fire detection and alarm system-Cargo spaces shall be fitted with either a fixed fire detection and fire alarm system or a sample extraction smoke detection system complying with the requirements of the FSSC. PSCO observed smoke extraction system in off-mode with fuse line detached from system. Once turned on, alarms failed on cargo holds 1-5. After E/O conducted maintencance on system, system would not show air-flow fault on 1,3,4, and 5 when valves where closed.
