Cargo tanks in which Grade A, B, or C liquids are to be transported shall be fitted with a pressure-vacuum relief valve (P/V). The P/V installed is relieving at a pressure of 0.5 psi on the vapor line when the P/V is set at 2.5 psi for venting. P/V appears to have significant wastage in the upper ring not allowing the device to properly seat and seal. Repair or replace P/V prior to loading any cargo that requires the use of the P/V but in no case more than 30 days from today.

All exposed and dangerous places such as gears and machinery shall be properly protected with covers/guards. The belt guard for the port prime mover engine is broken and not covering the belt. Repair or replace belt cover.
