Vessel reported a loss of power on 3/16/26 after a failure of the voltage regulator on the primary generator. Submit class report attesting to the repair of the generator by 25MAR2026.46 CFR 91.40-3(c)

On a vessel subject to STCW, the individual meeting the requirements of this section must also hold an STCW endorsement as Officer in Charge of an Engineering Watch with the appropriate propulsion power for the vessel upon which they are operating, except as noted in 15.105(g) for vessels on domestic near-coastal voyages. Licensed engineer does not hold appropriate STCW endorsement. 46 CFR 15.825(b)

A designated Duty Engineer (DDE) endorsement authorizes service as a Chief or Assistant Engineer on vessels of less than 500 GT in the following manners: When serving on a vessel to which STCW applies, the appropriate STCW endorsement must also be held. Crew member filling Chief Engineer position does not hold approprate STCW endorsement. 46 CFR 15.915(a)(3)

Onboard a passenger ship, as defined by the Convention for the Safety of Life at Sea, 1974, as amended (SOLAS), incorporated by reference, see 15.103, on an international voyage, any person serving as Master, Chief Mate, Mate, Chief Engineer, engineer officer, or any person holding an MMC and performing duties relating to safety, cargo handling, or care for passengers, must meet the appropriate requirements of Regulation V/2 of the STCW Convention (incorporated by reference, see 15.103). These individuals must hold documentary evidence to show they meet these requirements. Documentary evidence not provided for ship's crew. 46 CFR 15.1103(f)

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. PVC crush fitting found modified with sealants in the overhead of steering gear space. 46 CFR 116.415(a)(2)(ii)

Suitable means, such as a manual damper, automatic damper, or vent cover, must be provided in an accessible location outside the space served by the ventilation duct for shutting off the passage of air through the ventilation duct in the event of fire. Commercial laundry vent on lower weather deck was seized in the open position. 46 CFR 116.610(d)

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Breaker panel in crew laundry was missing a blank and allowed access to electrical shock. 46 CFR 120.200(b)

A door in a stairtower, stairway, and main vertical zone bulkhead must meet the following additional requirements: A door must be of the self-closing type capable of closing against a 3.5 list of the vessel. Port stairwell fire screen door on lounge deck did not fully close. 46 CFR 116.435(c)(6)(i)

A door in a stairtower, stairway, and main vertical zone bulkhead must meet the following additional requirements: Holdback hooks are not allowed. If installed, a hold back mechanism for a door must allow the door to be released: locally, upon a signal from a control space; and upon disruption of the power system. B class door on 300 mud room deck had permanent holdback only capable of local release. 46 CFR 116.435(c)(6)(ii)

All cable and wire must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling retention of multiple cable installations, and not used as a means of support. Wiring installed for new automation management found supported with zip ties. 46 CFR 120.340(b)(4)

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Dining room e-light above coffee alcove was inoperable. 46 CFR 120.432(a)

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The Company should ensure that each ship is: manned with qualified, certificated and medically fit seafarers in accordance with national and international requirements; and appropriately manned in order to encompass all aspects of maintaining safe operations on board. The company shall conduct an internal audit focused on their implementation of this provision in light of the 03 Coast Guard issued deficiencies detailing insufficiently credentialed crew being provided to the vessel.74SOLAS(20) IX/3.1 & ISM Code 6.2

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Vessel reported #1 and #2 generators are non-operational.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. Operator self reported both generators were not operational at the time of the inspection due to being serviced for required preventative maintenance. Once generators are fully operational, provide certification from Master and Chief Engineer to Sector San [name withheld] that both generators are fully functional and capable to taking all required loads. 46 CFR 115.804

All vessels shall be provided with a satisfactory bilge pumping plant capable of pumping and draining any watertight compartment except for ballast, oil and water tanks which have acceptable means for filling and emptying independent of the bilge system. Void space containing 3 separate bilge suctions (AA, H, C) found the C suction was inoperable. 46 CFR 56.50-50

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Tests of each rescue boat and each rescue boat launching appliance and survival craft launching appliance in accordance with § 122.520 of this chapter. Servicing report dated 25NOV24 from Survitec noted limit switch for both rescue boat launching arrangements require replacement. Provide report attesting to replacement of both limit switches by 26FEB2025. 46 CFR 115.808

Stowage of cylinders: Cylinder must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. Oxygen and acetylene bottles in the engine room found not properly stowed. 46 CFR 147.60

The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency, due regard being paid to such services as may have to be operated simultaneously. Emergency diesel generator day tank found filled to approximately 50% and determined to be insufficient to meet the time requirements associated with this regulation. 74 SOLAS (14) II-1/42.2

Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. 46 CFR 122.410(b)(2)

Each boiler, including superheater, reheater, economizer, auxiliary boiler, low-pressure heating boiler, and unfired steam boiler, must be available for examination by the marine inspector at intervals specified by Table 61.05–10. Both boilers onboard were due for fireside inspection on 30MAR24. Complete inspection by 23MAY24.

Auxiliary boilers and their associated piping and fittings will be given separate consideration and must meet the applicable requirements of subchapter F as determined necessary by the cognizant OCMI. All automatic auxiliary boilers must be tested and inspected in accordance with this subpart (61.35) and subpart 61.05 of this part. Each boiler must be available for examination by the marine inspector at intervals specified by Table 61.05-10 to determine that the complete unit is in a safe and satisfactory condition. Table 61.05-10 requires a hydrostatic test on watertube boilers in service on a Passenger Vessel twice in any five year period, with no more than three years between any test. The vessel’s Port Auxiliary Boiler and Starboard Auxiliary Boiler were last tested on 16OCT2018. Hydrostatically test the Port Auxiliary Boiler and Starboard Auxiliary Boiler to 1.25 MAWP prior to carrying passengers for hire.

Auxiliary boilers and their associated piping and fittings will be given separate consideration and must meet the applicable requirements of subchapter F as determined necessary by the cognizant OCMI. All automatic auxiliary boilers must be tested and inspected in accordance with this subpart (61.35) and subpart 61.05 of this part. Each boiler must be available for examination by the marine inspector at intervals specified by Table 61.05-10 to determine that the complete unit is in a safe and satisfactory condition. Table 61.05-10 requires boiler safety-valve tests twice in any five year period, with no more than three years between any test. The Port Auxiliary Boiler and Starboard Auxiliary Boiler safety valves are overdue for testing as of 16OCT2023. Test the Port Auxiliary Boiler and Starboard Auxiliary Boiler safety valves prior to carrying passengers for hire.

For a vessel, the construction of which was begun prior to approval of the plans and information required by paragraphs (a) and (b) of this section, the cognizant OCMI may require any additional plans and information, manufacturers’ certifications of construction, testing, and inspections, which the OCMI determines are necessary to verify that the vessel complies with the requirements of this subchapter. The vessel installed 480VAC Galley Equipment without Coast Guard Plan Approval. Submit the following plans to the Marine Safety Center for approval: One-Line Diagram, Cable List, Bill of Material, Type and size of cables, Rating and setting of overcurrent devices, and electrical plan load analysis.

Auxiliary boilers and their associated piping and fittings will be given separate consideration and must meet the applicable requirements of subchapter F as determined necessary by the cognizant [name withheld] auxiliary boilers must have MSC approved test procedures with detailed instructions for operationally testing each automatic auxiliary boiler, its controls, and safety devices. The vessel’s PSTPs did not accurately reflect the test methods for the specific control system requirements found in 46 CFR 63.20-1 to include: Primary safety control system, combustion control system, water level controls and low water cutoff controls. Submit revised test procedures to MSC for approval.

Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. The monitoring device(s) must: Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive; Remain operable during the nighttime watch and; Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Vessel does not have monitoring devices in place for the wakefulness of watchmen and has not submitted plans to the Coast Guard.

On vessels described by 46 CFR 114.110(e), flammable items not covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Develop written procedure for any operation and/or storage of potentially hazardous items onboard including small lithium ion batteries and their charging arrangements. 46 CFR 122.364

The vessel operator shall keep a written record available for inspection by the COTP or OCMI of: The Declaration of Inspection as required by 156.150(f) of this chapter. DOI’s were not available for inspection.

A door in a stairtower, stairway, and main vertical zone bulkhead must meet the following additional requirements: A door must be of the self closing type capable of closing against a 3.5 list of the vessel. Main deck aft stairtower A door did not fully close.

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The port day tank fuel shutoff valve was found to be inoperable. The valve was repaired and tested satisfactorily in the presence of the attending Marine Inspector.

All unfired pressure vessels must meet the applicable requirements of subchapter F. If your vessel’s Certificate of Inspection is renewed annually, the marine inspector must c heck the settings of the safety or relief valves on all pressure vessels at each inspection for certification. The Port Engine Room Air Receiver safety valve was found to not lift at the pressure vessel’s Maximum Allowable Working Pressure (MAWP). The pressure gauge was replaced, and the safety relief valve was retested in the presence of the Marine Inspector. The safety relief valve lifted at the MAWP, and limited any further rise of pressure.

Paint and flammable liquid lockers must be constructed of steel or equivalent material, or wholly lined with steel or equivalent material. Paint lockers on board could not be closed and secured as originally designed. 46 CFR 116.405e

The design, construction and stowage of portable tanks and related fuel lines and accessories must meet the requirements of ABYC H-25, “Portable Gasoline Fuel Systems for Flammable Liquids,” or other standard specified by the Commandant. Marine Inspector identified four portable gasoline fuel tanks used for zodiac operations have UV damage and delamination. 46 CFR 119.458(b)

A fixed fire detection system shall be so installed and arranged to provide smoke detection in service and accommodation spaces including corridors, stairways and escape routes. Smoke detector in forward port crew stairway not on fire control plan. SOLAS CONSOL 2014, Ch II-2, 5.5.2.

Doors in A-Class fire control boundary, if a main vertical zone bulkhead, meet the following additional requirement-be self closing against a 3.5 degree list of the vessel. 46 CFR 116.435 (c) (6) (i)

Replace five AFF fire extinguishers as ID'd prior to carrying passengers. 46 CFR 118.500

Provide liferaft service reports prior to carrying passengers. 46t CFR 117.200/1122.730(a)(1)

Fire control plan must indicate life ring locations as ID'd. 46 CFR 117.70/46 CFR 116.202(b)(7)

Submit fire and safety plan to reflect accurate fire and lifesaving equipment. SOLAS 2014 CONSOL, Ch II-2, Reg 15, 2.4.

#1 AUX Gen had an overspeed condition and was shutdown. Prove proper operation of the #1 AUX GEN to the satisfaction of USCG Marine Inspector.

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: the fire integrity of the divisions shall be maintained at openings and penetrations. Two self closing class A doors (sundeck stair tower door across from bar and main deck access to crew berthing) failed to properly close. 74 SOLAS (14) II-2/9.1.3

No ship to which the present Convention applies shall proceed to sea on an international voyage after the date on which the present Convention comes into force unless it has been surveyed, marked and provided with an International Load Line Certificate (ILLC). ILLC is expired. Provide valid ILLC prior to sailing international. ICCL 1988 Annex A Article 3

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including operational test of the means provided for pumping bilges. Vessel crew unable to demonstrate operation of bilge system in each space. 46 CFR 115.804(h)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. Vessel COD is expired and no valid COD is currently on board. Vessel route is reduced to Coastwise domestic voyages only until resolved. 46 CFR 67.313
