Pressure relief valves should be tested and set and sealed by a competent authority acceptable to the administration on a record of this to be maintained on board. LR issued certificate for valves was intended for ship number 5756 of Hyundai Samho Heavy Industries. This vessel is 5758, Serial numbers for cargo tank relief valves do not match documentation on board. Prior to cargo operations provide USCG Houston proper set and seal documentation for cargo tank relief valves. Within 30 days the company should complete an internal investigation in accordance with the procedures in ISM 9 to investigate and analyze this deficiency and implement corrective actions including measures to prevent reoccurrence.
