Fire protection system and fire fighting systems and appliances shall be maintained ready for use. PSCO oberved (1) Zone 6, cable/short circut fault on the fire dection system on the bridge. All hatchways in position 1 and 2 shall be fitted with hatch material, except as provided in reg 14(2). such covers shall be weathertight and fitted with gaskets and clamping devices. PSCO observed multiple ventilation hatches dogs missing on the watertight hatches on deck and cargo ventilation hatches. A requirement was issued to provide a class report attesting to the replacement of the missing dogs on all cargo ventilation covers and watertight hatches on deck prior to departure. The GMP shall also designate the person or persons in charge of carrying out the plan. C/O stated he is in charge of the plan but could not show where it was documented in GMP. MARPOL Annex V 10.2, 40ac

The GMP shall provide written procedures for minimizing, collecting, storing, processing, and disposing of garbage, including the use of the equipment onboard. Vessel GMP does not list the processing equipment or address the use of onboard equipment. MARPOL Annex V 10.2, 40ac

For this purpose, the following functional requirements shall be met, 3. the limitability of combustible materials shall be restricted. PSCO observed excessive oil and oily rags in the bilge beneath #3 generator. In addition, test and inspections shall be conducted to determine the service ability of all protective enclosures and devices fitted to the location. PSCO observed a fault reading “channel 11” on the VDR. A restriction was issued to provide a class report attesting to a fully operational VDR with no alarms. 74SOLAS2020 V/18.8, 17ac

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the authorized ballast water management methods listed in 33 CFR 151.2025(a).  Vessel’s ballast water management system is inoperable.  Vessel has been granted an extraordinary circumstance to employ the ballast water management method listed in 33 CFR 151.2025(a)(3) to conduct cargo operations while in the Sector New Orleans COTP Zone.  In accordance with 33 CFR 151.2040(b)(3), the vessel must discharge only that amount of ballast water operationally necessary to ensure the safety and stability of the vessel for cargo operations.

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. 29 out of 32 immersion suits are unserviceable due to unsealed seams. 74 SOLAS (20) III/20.2

The company and the ship shall comply with the requirements of the international Safety Management Code. 1730- Oily-water seperating equipment- Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with alarm arrangements to indicate when the level cannot be maintained. The system shall also be provided with arrangements to ensure that any discharge of oily mixtures is auto-stopped. Crew could not prove proper operation of oily-water seperator- MARPOL I/14.7
