The port engine room pre-engineered fixed gas fire extinguishing system override function did not work.

The ventilation closure for the starboard engine room did not fully seal the opening.

Replace the Type I retroreflective material on 15 adult life jackets as identified.

Place the vessel's name on the ring life buoy.

Penetrations of required watertight bulkheads by any rigid plastic pipe are prohibited unless the following requirements are met: i) Each penetration must be accomplished using an acceptable metallic through bulkhead fitting, and ii) On or more metallic shutoff valves must be installed adjacent to the fitting in on of the following ways: A) Only one metallic shutoff valve must be installed if it is operable from above the bulkhead deck, or B) If two metallic shutoff valves are installed, one on either side of the bulkhead, they need not be operable from above the bulkhead deck provided immediate access to both is possible. -The vessel has PVC grey water penetrations in the port engine room forward bulkhead which is considered watertight. Address at next drydock exam.

Make starboard engine room bilge high level alarm operational.

Clearly mark the name of the vessel on each ring life buoy.

Secure the marine sanitation device three way valve with a padlock or wire-tie or by removing the handle.

Provide educational information as part of the company's required Employee Assistance Program. The educational information shall include the display and distribution of a community service hot-line telephone number for crewmembers seeking assistance with drug and alcohol abuse.

Provide and document training for all crewmembers as part of the company's required Employee Assistance Program. The training information shall include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment as well as the manifestations and behavioral cues that may indicate drug and alcohol use and abuse. Training for personnel in supervisory positions (Captains) must receive at least 60 minutes of training.

Vessel's pre-engineered fire fighting system failed to restart the generators and engines in the override position. The switch was replaced and the override function worked properly.

Hydrostatic Release for the EPIRB was expired and replaced.

Bilge strainer in Port lazarette was broken off of pipe and laying in the bilge. Ensure strainer is properly installed at the termination of bilge line in port lazarette pick up.

Fiberglass delaminated on goose-neck. Submit repair plan proposal to local OCMI for approval prior to repairs completed.

Aft hatches are not permanently affixed/attached to the deck. Reattach aft deck hatch covers.

Provide up to date publications in print formate, to include the Coast Pilot [name withheld] and Tides.

Make foresail rigging available for inspection (lift furling units) by local Coast Guard Marine Inspector.

Repair to Gooseneck Sheduled to be finished by 14 Feb 2020.
