Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed both steering gear motor controllers not able to have power secured as designed. The linkage from the external switch and the breaker was removed and power was secured only by unlocking the cabinet and using an adjustable wrench to change the breakers position.

PSCO observed lagging soaked with fuel oil in many locations in particular the purifier room and lagging at the bottom of the HFO tanks and associated piping.

When the fire-extinguishing medium is stored outside a protected space, it shall be stored in a room which is located behind the forward collision bulkhead, and is used for no other purposes. Any entrance to such a storage room shall preferably be from the open deck and shall be independent of the protected space. PSCO observed emergency generator CO2 room door will not close due to a warped door.

PSCO noted the vessel failed to report several non-conformities, per vessel's Safety Management System (SMS)procedures, of a technical and operational nature which collectively provide objective evidence of a serious failure or lack of effectiveness of the implementation of the ISM Code. Due to the failure or lack of effectiveness of the SMS, an external SMS audit shall be carried out.
