The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: Fire detection and fire alarm systems. PSCO observed trash bags covering the #3 generator flame detector and smoke detector. Ship's crew removed trash bags from flame detector and smoke detector and tested satisfactory by PSCO. 74 SOLAS (20), II-2/14.2.1, Deficiency Rectified, Code 10c.

Resolution: The deficiency was cleard on the spot. Crew removed bags and detectors were tested the the satisfaction of the PSCO.

Machinery spaces of Category A above 500 m^3 in volume shall, in addition to the fixed fire extinguishing system required in paragraph 5.1.1. be protected by an approved type of fixed water based or equivalent local application fire extinguishing system based on the guidelines developed by the organization. Hypermist system was found in "local" operating position. Hypermist control panel stated control mode to be placed in "remote" position. Crew switched the system to the remote "setting". 74SOLAS(14)II-2/10.5.6.2

Resolved 8 August 2018
Resolution: Crew returned selector switch to proper "remote" setting as set forth by operating procedures of hypermist system. Educated crew on importance of ensuring that this is done after maintenance is completed.

At Approx 2000 on 19Mar2017, vessel reported dragging anchor and ultimately ran aground with sandy shoal in the vicinity of Crims Island.
