All piloit ladders used for pilot transfer shall be clearly identified with tags of other permenent markings so as to enable identification of each appliance for he purpose of the survey, inspection and record keeping. A record shall be kept on the ship as to the date the identified ladder is placed into service and any repairs effected. PSCO observed the pilot ladder service record did not match the serial number on the identification plate on the pilot ladder. 00 pilot ladders on board have matching certificate.

Safety management objectives of the company should, inter alia: provide for safe practices in ship operation and a safe working environment. Last night, following an issue where crane grabber would not open, the crew troubleshot the problem and determined the plug connection to the solenoid valves was bad. To save time, the crew changed the plug connections from "slow" open to normal open. Shortly thereafter, the crane grabber opened without input from the remote operator and dumped gypsum on a truck cab causing minor injuries to the driver and damage to the truck.
