15104 - Masters responsibility and authority - The company should establish in the safety management system that the master has the overriding authority and the responsibility to make decisions with respect to safety and pollution prevention and to request the company's assistance as may be necessary. On 20Jun2020, during cargo operations, vessel broke away from the dock. PSCO observed that the vessel's Safety Management System (SMS) states that "the ship must have sufficient and efficient mooring lines sent out to secure her fast to the wharf or buoys at all times." Additionally, the SMS states that the Master is responsible for implementing this procedure. Master was aware of the inadequate mooring arrangement at this dock as the vessel had previously moored at the same location and the Master provided a Letter of Protest he had drafted at that time regarding the dock arrangement. Company to provide a corrective action plan with concurrence from Administration.
