The machinery associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces, and other hazards. PSCO observed leaks on Generator #3 from the cylinder #6 resulting in collection of fuel oil in the overflow drainage area. Upon further investigation, the #3 G/E leak drain tank high level alarm was found inoperable caused by the XP02 Control Processor failure. Resulting in no functional alarms on all three generators.
Action required: 60 - Rectify deficiencies prior to movement

The purpose of this regulation is to prevent the ignition of the combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed active leaks on incinerator pump fuel lines.

The condition of the ship and it’s equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed a leak on hydraulic lines located above the rope storage door on top of the A/C vents. Vessel had a flange wrapped in cloth with a collection bucket underneath.

Resolution: Rcvd, reviewed and accepted class report stating that the crew replaced the O-rings and system was tested under working conditions to satisfaction of class.

PSCO observed inaccurate soundings of the sludge tank. PSCO could not verify the soundings of the sludge tank against the oil record book and sounding table.
