Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement. MI observed chaffing on fuel lines in the Engine Room. 46 CFR 182.455 (b)(3)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. There was a fuel leak found in the centerline void space. Clean fuel. 46 CFR 182.100

Electrical equipment on a vessel must be installed and maintained to.. minimize accidental personnel contact with energized parts. Identified several dead end wires without caps/tape to in the console of the upper pilothouse. 46 CFR 183.200

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire. No instructions posted for local operation of the fire suppression system. 46 CFR 185.510 (a)

A vessel must carry one floating waterlight attached to the ring life buoy. The battery in the light for the ring life buoy was expired. 46 CFR 180.70 (d) (1)

Ship station license present for operation of all maritime services transmitting equipment on board a vessel. FCC Radio telephony certificate was expired. 47 CFR 80.13 (b)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificates onboard were expired. 46 CFR 10.301

A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12. Medications in the first aid kit were expired. 46 CFR 199.175 (b)(10)(i)

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Vapor tight boundary is not maintained due to a hole in void aft of the engine room. 46 CFR 177.405(c)

Electrical equipment on a vessel must be installed and maintained to.. minimize accidental personnel contact with energized parts. Identified three dead end wires without caps/tape to prevent from accidental electrical shock. 46 CFR 183.200

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Identified expired drug alcohol swabs on board. 46 CFR 4.06-15

All wiring and cables shall be suitable for marine service and for particular installation used. Dead-end electrical wires are located in multiple spaces. 46 CFR 183.10-20(a)

A hatch cover must have securing devices. Hatch located in forward crew accommodation does not have securing devices. Effect repairs to the satisfaction of the attending Marine Inspector.

At each initial and subsequent inspection for certification and re-inspection the inspector shall examine and test all items in the ship's outfit, such as ground tackle, navigation lights, compass, etc., which are required to be carried by the regulations in this sub-chapter. Navigation lights are inoperable. 46 CFR 176.25-40(a)

Each pressure type wire connector and lug must meet UL 486A (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. The use of twist-on type wire nuts is permitted under the following conditions: (1) The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration; and (2) Twist-on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit - MI found a loose wire in the vicinity of the bilge space underneath port side deck. Request owner/operator to secure the wire IAW 46 CFR 183.340(i)(1).

Electrical equipment on a vessel must be maintained and installed to protect passengers, crew, and other persons and the vessel from electrical hazards. Vessel had several loose wiring from inoperable electrical equipment and dead end wires. Vessel shall clear all dead end wiring, disconnected from power sources, and make all possible attempts to remove the wiring from the vessel.

A vessel on limited coastwise route must carry six red hand flare distress signals and six orange smoke signals. Vessel has seven red flares and 2 orange smoke signals. 46 CFR 180.68(a)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette; (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. Vessel's visual indicators for high level bilge alarms are not operable. 46 CFR 182.530(a)

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Vessel has a flex hose penetrating the second forward-most watertight bulkhead without any watertight sealant. 46 CFR 179.320(a)

A vessel of more than 26 feet in length, but not more than 65 feet, must carry a minimum of one life buoy of not less than 24 inches in diameter. Each ring life buoy must be approved in accordance with 160.150 in subchapter Q of this chapter. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Vessel's ring life buoy is not in a serviceable condition, including the grab line, which are significantly deteriorated. 46 CFR 180.170(a)(2) 46 CFR 180.170(b)(1) 46 CFR 160.050-3(c)

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Vessel's IBA painter was not connected to the float free link. 46 CFR 180.130(a)(1)

A carbon dioxide cylinder may not be inclined more than 30° from the vertical unless fitted with flexible or bent siphon tubes, in which case it may be inclined not more than 80° from the vertical. Vessel's fixed firefighting carbon dioxide bottle is inclined past 30° and does not have an indication of being fitted with a siphon tube. 46 CFR 181.410(c)(7)

A vessel on a limited coastwise route must carry six red flares approved IAW 160.021 and six orange smoke distress signals approved IAW 160.037. Vessel is missing 3 red and 3 orange flares or approved substitutions. 46CFR180.68

Each space accessible to passengers or used by crew on a regular basis must have at least two means of escape. Emergency escapes in aft berthing areas overheads have no ladders or means for a passenger to climb out. Provide means for passenger to climb out of emergency escapes. 46 CFR 177.15

Wiring where subject to mechanical damage shall be protected IAW the National Electrical Code. Wire run for engineroom high water alarm, engineroom U-lube system, wire in aft engine room above the racor assembly are not secured and installed properly creating the possibility for wire chaffing. 46 CFR 183.05-50 (a)(2)

Penetrations and openings in watertight bulkheads must have a means to make them watertight. Port Engineroom bulkhead has through hull penetration that is not watertight. 46CFR171.114(a)(2)

All wiring and cables shall be suitable for marine service and for the particular installation used. Lighting cable located starboard side forward bulkhead next to entrance of engine room has exposed splice utilizing twist connectors. 46 CFR 183.10-20

At each vessel inspection, hazardous situations must be corrected. Vessel sign limiting 11 passengers to upper deck is missing. 46 CFR 184.502

A vessel must comply with the applicable requirements for any radio including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Vessel is missing FCC License and Safety Certificate. 46 CFR 184.502

As appropriate for the intended voyage, a vessel must carry up to date charts, coast pilot, light list, and tide tables. Charts, Light List, and Coast Pilot are not current. 46 CFR 184.420 (a)

The installation of a fire extinguisher must be to the satisfaction of the Officer in Charge, Marine Inspections. Galley fire extinguisher and bracket is not mounted. 46 CFR 181.520

On a vessel engaged on excursion type operations, rails shall be fitted with bulwarks, chain link fencing, wire mesh, or the equivalent. Rail courses have gaps near 12 inches near bottom and top courses of rail. 46 CFR 177.35-1 (c)

All machinery installed must be maintained to afford adequate protection from machinery failure. Both ram seals leaking on steering ram. 46 CFR 182.01-5

Suitable rails shall be installed in way of all exposed hazards. Port side rail for upper deck ladder is loose where it mounts to the helm dash. 46 CFR 177-35-15

At each inspection, the marine inspector shall require hazardous situations to be corrected. Port Window 4th aft from bow is cracked.
