Cargo tanks shall be fitted with one of the following types of gauging devices: Closed device: which penetrates the tank, but which is part of a closed system and keeps tank contents from being released. PSCO observed high pressure vapors being released from the pluggers used to test the high level and overfill level alarms for all the cargo tanks on the deck releasing flammable and toxic vapors everytime the alarms are manually tested on deck. 74SOLAS(14) VII/10.1 IBC Code 13.1.1.3

Resolution: Class Survey Record confirmed by maker's manual that it is possible for cargo vapor during the test of high level and overfill alarms to leak from the plunger apertures.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed o sea without danger to the ship or persons on board. PSCO observed painted-over soft patch on potable water line aft of frame 55 on deck actively leaking. 74SOLAS(14) I/11(a)

Resolution: Vessel submitted satisfactory class report attesting to permanent repairs of the portable water line.

Issued 5 November 2021 Resolved

Cargo tanks shall be fitted with a visual and audible high-level alarm which complies with IBC Code 15.19.1 to 15.19.5 and which indicates when the liquid level in the cargo tank approaches the normal full condition. PSCO observed the overfill alarm (98.5%) activate and only sound a brief notification and only a brief activation of the red light. After initial notification, all alarms and notifications cease to exist. Rectify deficiency prior to loading cargo. 74 SOLAS (20) VII/10.1 - IBC Code 15.19.6

Resolution: Alarm system was reset for overfill alarm and retested to satisfaction of PSCO.

Self-activating smoke signals required by SOLAS regulation III/7.1.3 shall: be provided with a quick-release arrangement that will automatically release and activate the signal and associated self-igniting light connected to a lifebuoy having a mass of not more than 4 kg. PSCO observed smoke signal attached directly to the vessel and would not self ignite when the ring buoy was released. 74 SOLAS (20) III/34 - LSA Code 2.1.3.6

Resolution: Smoke signal was reattached to ring buoy and unattached from vessel, allowing for proper self igniting function.

An immersion suit shall be constructed with waterproof materials such that it can be unpacked and donned without assistance within 2 minutes. PSCO officer observed, Second Officer was unable to don immersion suit within time requirements and unassisted. It took the Second Officer more than 5 minutes to don immersion suit with the assistance of another crew member. LSA CODE/CH II-2.3.1.1 (17a/c)

Fire Fighting › Fixed Pressure Water-Spraying Fire Extinguishing System › Valves

0725- Fixed Fire Extinguishing Installation- Water spray sys shall be inst to cover parts of fuel strg tank. PSCO identified 01 damaged in-line supply valve for the methyl alchol tank fixed sprinkler system. Valve would not open when activated and water was unable to flow through the sprinkler heads. 74 SOLAS Amend. (2014) II-1 Reg 57, IGF Code 11.5.1 17a.
