The fire detection systems shall be kept in good order so as to ensure the required performance if a fire occurs. The smoke detector above the fuel oil purifiers did not trigger either audible or visual alarms.

Means shall be provided to control leaks of flammable liquids and ignitability of combustible materials shall be restricted. Wooden wedges were found braced in fuel oil purifier, oily rags were found on main engine and service generators and other machinery spaces, oil leaks were found on main engine and service generators.

After any survey of the ship has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey. The isolation valve installation on the sight glass for the diesel fuel oil service tank was mechanically kept in an open position.
