In reference to deficiency #02, PSCO observed and reviewed documents for new lights for inoperable fixtures on the ship. Deficient item was reported to company in October. Vessel and company failed to restore used and missing lights for over 02 months in addition, vessel failed to follow shipboard manuals and guidance for the deficiencies listed as 03, 04 and 05. An additional internal audit is required.

Each vessel vapor connection flange face must have a permanent stud protruding outward that has a 12.7mm (0.5 inch) diameter and is at least 25.4mm (1 inch) long.
PSCO observed both vapor control flange connections on the port side and 01 on the STBD side missing the required stud on the flange face.
