A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. PSCO received report that vessel suffered blackout on 30 March 2026. Upon arrival to vessel captain stated generators #1 and #3 experienced low frequency alarms and caused blackout. A root cause analysis is required to be submitted to our office for review.

02101 - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Vessel reported to Sector Maryland that while opening the aft cargo door, a hydraulic hose ruptured rendering the cargo door in-operable.

Oil filtering equipment shall be of a design approved by the administration and will ensure that any oily mixture discharged into the sea has an oil content not exceeding 15 ppm. 15 ppm panel reads "Dessiccator Worn Out" when attempting to test alarm. Vessel shall not operate oily water separator (OWS) until rectified.
