Marine inspectors observed exposed exhaust components on port and starboard engine turbo assemblies subject to temperatures greater than 150 degrees to be missing insulation. 46 CFR 177.970

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Marine inspectors observed excessively chaffed areas of both port and starboard engine cooling hoses that resulted in exposed secondary wire layer. 46 CFR 182.100

All equipment, including switches, fuses, lampholders, etc., must be suitable for the voltage and current utilized. Marine inspectors observed the onboard generator data plate stamped temperature rating of 40 degrees celsius. Vessel engineering space is not equipped with power ventilation and may be susceptible to temperatures exceeding the listed generator maximum output temperature rating, MIs also observed a main breaker rating of 90 amps. Vessel must provide objective evidence to show that the installed generator and main breaker rating is capable of performing the allowable output at the temperatures experienced within the engineering space without causing undue stress to the installed electrical components. 46 CFR 183.220(b)

In systems and applications where flexible hoses are permitted to be clamped: (1) Double hose clamping is required where practicable. MI observed single hose clamps on cooling water hoses for both main propulsion engines.

The owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection of all accessible parts of the exterior and interior of the hull. Found previous repairs and cracks in transom which have failed allowing for minor water ingress. Conduct necessary repairs of the transom and provide mitigation for future occurrence. 46 CFR 176.802(a)(1) 50 c

A vessel must comply with the structural design requirements of the standards for aluminum hull vessels. Cracks were observed on multiple frame welds in the aft lazarette space. Submit repair plan for review by lead marine inspector. 46 CFR 177.300(d) 50 c

Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. Observed steering hydraulic hose showing chafing/wear. Conduct necessary repairs and prevent future chafing. 46 CFR 182.720(e)(3)(ii) 16 c

All cable and wire must be protected by metal coverings or other suitable means if in areas subject to mechanical abuse. Electrical wire in Engine room is showing sign of chafing. Conduct necessary repairs and prevent future chafing. 46 CFR 183.340(b)(6) 16 c

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Numerous deck railings on the vessel were found significantly cracked at the base. Conduct necessary repairs. 46 CFR 177.900(b) 16 c

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirement. Observed both forward engine mounts on both main engines to be cracked. Cracks on the engine mounts were noted in prior inspections. Given the repeated nature of this matter, submit repair plan to include means of preventing future damage. 46 CFR 182.200(a) 50 c

In the amidships void space beneath the pilothouse, found vertical stanchion with top welds cracked. Repair as initially designed to satisfaction of USCG. 46 CFR 176.802(a) 16 c

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. Fire boy engine shutdowns were not operating as intended; repair to the satisfaction of the attending marine inspector.

Repair through hull fittings where the electrical cables pass from the engine room through the void under the pilot house. There must be a vapor tight seal.

Repair oil leak on the Port main engine.

Repair air hose connection on the Port main engine.

Found multiple exposed wires under operating station console. Electrical equipment on a vessel must be installed to protect crew, other persons, and the vessel from electrical hazards. Corrected on the spot to the satisfaction of attending marine inspector.

Found multiple connectors under operating station console that utilized normal spade connectors. Wire connectors utilized in conjunction with screw type terminal block must be of the captive type. Corrected on the spot to the satisfaction of attending marine inspector.
