The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system. Upon activation of the emergency generator the sliding fire screen door (2-FD1 Sliding NYE STBD) failed to close due to a faulty battery installation in the overhead. Provide email statement attesting to the renewal of the battery and proper operation of the sliding fire screen door.

In the event of an accident to a boiler, unfired pressure vessel, or machinery tending to render the further use of the item unsafe until repairs are made, or if by ordinary wear such items become unsafe, a report shall be made by the chief engineer immediately to the Officer in Charge, Marine Inspection. Vessel reported oil/water leak from the combining gear cooler. Vessel must complete repairs to the satisfaction of a USCG Marine Inspector.

Resolution: The cooler had a pin hole in one of the tubes and was replaced. The sw pump is normally started before the lo pump which allowed water in the lo system. The lo system runs at a higher pressure, the lo leaked into the sw system which ran overboard.
