Alarms for gas sampling systems should be activated when the vapor concentration reaches the threshold limiting value. During testing of the ammonia sensor while the ship was discharging ammonia cargo, the sensor was unable to be calibrated by the crew. Crew replaced sensor and conducted calibration in accordance with manufacturer's instructions. PSCO witnessed successful calibration. Recommend third party calibration in accordance with company policy as soon as practicable.

Resolution: Corrected on the spot. Crew installed new sensor and and conducted satisfactory calibration in accordance with manufacturer's instructions.
The machinery shall be of a design and construction adequate for the service for which they are intended. Cylinders #2 and 3 failed due to low voltage in software module on main machinery installation. Provide class report attesting to proper operation prior to departure.
