Oil fuel pipes, which if damaged would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO discovered quick closing valves of HFO Service tank to main engine, generators 1,3, and 4 did not close fully.

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. PSCO found that the emergency generator weekly checklist did not include procedures for manual testing of the emergency shutoff valve. Ship is required to conduct an internal audit and take an internal audit within three (3) months due to the procedures for checking emergency generator not sufficient to prevent operational readiness.

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met. Fire protection systems and fire-fighting systems and appliances shall be maintained and ready for use. PSCO discovered the emergency shutoff valve of the storage tank for the emergency generator was in a closed position.
