The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and/or operational nature of deficiencies 02, 03, 04, 05, and 06 provide objective evidence of a serious failure of the implementation of the ISM code. An external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency. 74SOLAS(20)IX/3.1 ISM Code 10.1

After any surveys of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in the structural arrangements, machines, equipment, and other items covered by the survey without the sanction of the Administration. PSCO observed improper modification of 3 inch wooden pegs installed in the fuel system (in way of fuel injection throttle device) for cylinders 1 through 5 of the M/E. 74SOLAS(20)I/11(b)

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control the leaks of flammable liquids; means shall be provided to limit the accumulation of flammable vapours. PSCO observed an excessive accumulation of oil to include leaks in the machinery space and purifier room and in way of shaft seal. 74SOLAS(20)II-2/4.1.1 & 1.2

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. PSCO observed A-60 boundary door to incinerator room is not self-closing. 74SOLAS(20)II-2/9.4.2.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ships in all respects will remain fit to proceed at sea without danger to the ship or persons on board. PSCO observed material failure of hydraulic hose on stern ramp during lowering evolution. 74SOLAS(20)I/11(a)

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed dead-end wires in the purifier room for the auxiliary engine purifier as well as frayed wiring for the M/E purifier actuator. 74SOLAS(20)II-1/40.1.3

All ships of 300 gross tonnage and upwards and passenger ships irrespective of size, shall in addition to meeting the requirements of paragraph 2.2, be fitted with an echo-sounding device, or other electronic means, to measure and display the available depth of water. PSCO observed low power supply alarm. 74SOLAS(20)V/19.2.3.1

The liferaft shall be packed in a container that is... the container shall be marked. PSCO observed no legible markings on 6 person starboard liferaft. 74SOLAS(20)III/34 LSA Code IV/4.2.6.3

Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation... The system shall also be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 ppm. In considering the design of such equipment and approvals, the administration shall have regard to the specification recommended by the Organization. Ship staff training should include familiarization in the operation and maintenance of the equipment. PSCO observed when testing 15 ppm alarm water leaking from OWS filter tanks due to improper installation of filtering equipment. MARPOL Cons. (22) Annex 1 Reg 14.7
