When liquid cargoes other than those referred to in paragraph 6.1...additional safety measures shall be required, having due regard to the provisions of the International Bulk Chemical codes defined in regulation VII/8.1, the Bulk Chemical Code. The conditions of the ship and its equipment shall be maintained to conform with the provisions of the code to ensure the ship will remain fit to proceed to sea without danger to the ship or persons on board or without presenting an unreasonable threat of harm to the marine environment. Barge's Inert Gas system was inoperable preventing tests from being conducted. Conduct tests in the presence of a Marine Inspector. 74SOLAS(2020)II-2/6.2 IBC Code 1.5.3.1

Each portable container of flammable or combustible liquid used for portable auxiliary equipment must be stowed in a paint locker or an open location designated by the master. The "Alcohol" flame locker doors were not latching secure. 46 CFR 147.45(f)

The inspection shall be such as to insure that the vessel, as regards the...electrical installations..., is in satisfactory condition and fit for the service for which it is intended. Vessel was unable to test the reverse power relays for the generators as required by 46 CFR 111.12-11(f). 46 CFR 31.10-15(b)

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have onboard an emergency means to enable the person in charge of a transfer operation to a facility, to another vessel, or within the vessel to stop the flow of oil or hazardous material. Vessel was unable to test the emergency stop for the aft electric cargo pump (designated for phenol). 46 CFR 155.780(a)

The last 1.0 meter of vapor piping upstream of the vessel vapor connection...must be-painted in the sequence of red/yellow/red. The width of the red bands must be 0.1 meter and the width of the middle yellow band must be 0.8 meter; and labeled with the word "VAPOR" painted in black letters at least 50.8 mm (2 inches) high. MI observed the vapor collection system for the phenol cargo piping was not appropriately marked. 46 CFR 39.2001(h)

The inspections and tests shall insure that the equipment and associated structure are in satisfactory operating conditions and fit for the service in which they are intended. MI observed the handle for IG system valve HV-6.31 was significantly corroded and had multiple holes, compromising its integrity. 46 CFR 31.30-1 46 CFR 61.01-1(b)

Where firefighting equipment is not required, but is installed, the equipment and its installation shall be of an approved type. Annual fire fighting equipment servicing conducted NOV 2022 has outstanding inoperable equipment (alarm pull station and horn). Repair and/or replace units and verify installation with approved technician. Provide technician report to USCG.

Vessels carrying toxic of flammable products or both shall be equipped with at least two instruments designed and calibrated for testing for the specific vapors in question. Vessel's secondary means for testing toxic vapors(dragger tubes) were expired. Vessel is prohibited from loading Aceton or Phenol until providing proof of receipt of new dragger tubes.

When Table 1 in 46 CFR 153(Subchapter O) refers to this section, a cargo containment system must have a permanent inert gas system(IGS). The vessels CAA list n-Propylamine as a authorized cargo with a special requirement for a IGS. Vessel personnel could not prove proper operation of the installed inert gas system. Vessel is prohibited from carrying n-Propylamine until proper operation of the IGS is proven to the satisfaction of the USCG.

Bergan tank level and alarm monitoring system testing incomplete at delivery. System to be serviced by naufacture/vendor service representative. At delivery the following discrepancies were observed: cargo tank #1 port and starboard high level and overflow alarms, cargo tank #3 starboard temperature detector, cargo tank #4 port high level and overflow alarms, cargo tank #4a starboard high level and overflow alarms, cargo tanks #1 starboard and #2 port and starboard temperature sensors malfunctioning. Ballast tank level scale (~ 67' DEPTH) was incorrect. Diesel fuel oil tank level indicator malfunctioning. Ballast tank #2 port and forepeak appear to be reversed. When complete, contact cognizant OCMI, with CG Marine Inspector to attended and verify the Bergan tank level and alarm monitoring system. 46 CFR 39.2003 and 46 CFR 39.2009

Cargo tank #4 port, at delivery deep well suction valve stem was found bent. Valve be repaired or replaced and tested to satisfaction of attending ABS Classification Surveyor. 46CFR32.50-15(b)

Cargo tanks #1 port and starboard, at delivery Acetone cargo pump explosion proof electric motor was not installed. Install Acetone cargo pump explosion proof electric motor. When complete, contact cognizant OCMI, with CG Marine Inspector to attend, verify installation, confirm explosive rating and test of emergency shutdown. 46CFR32.50-1 & 46CFR32.50-3

Cargo tanks #4 and #4a's, Phenol cargo pump explosion proof electric motor not installed at delivery. Install the Phenol cargo pump explosion proof electric motor. When complete, contact cognizant OCMI, CG Marine Inspector to attend, verify installation, confirm explosive rating and test of emergency shutdown. 46CFR32.50-1 &46CFR32.50-3

Several discrepancies were noted on the Volcanic Thermal Glycol Heating System by attending ABS Class Society Surveyor during initial delivery. The Volcanic Thermal Fluid Heater shall not be operated until confirmation is provided and received from an attending ABS Surveyor that all outstanding items of class (OSR's) and discrepancies noted in ABS Sturgeon Bay initial survey new construction report SB2947709 were tested and examined to the satisfaction of the class surveyor. 46CFR63.10-1 & 46CFR 63.15-1

Cargo tank #4a port and starboard thermal fluid heating expansion loops wsere not properly supported with sufficient pipe hangers at delivery. It is recommended that appropriate pipe supports be added to the mid-point of the cargo tank 4a port and starboard heating expansion loops. Sufficient hanger to be added and examined to the satisfaction of the attending ABS Class Surveyor.
