Personal equipment shall consist of the following protective clothing of material to protect the skin from heat radiating from the fire and from burns and scalding from steam. Two of the vessel's four fire-fighting outfits have started to flake and are losing their protective coating. 74 SOLAS (14) II-2/10.1.1 - FSS CODE 2.1.1.1 17(c).

The owner/operator of any vessel equipped with ballast tanks that operates in the waters of the US must maintain a BWM plan that has been developed specifically for the vessel and allows those responsible for the plans implementation to comly with the requirements of this subpart (33 CFR 151 subpart D). Additionally, the master and crew must be trained on ballast water management procedures.
Ships ballast water management plan makes no mention whatsoever of US regulations, and ships crew has received no training or education on US ballast water requirements.
Submit USA specific appendix of BWMP and proof of onboard training for ship's crew regarding US ballast water requirements to [email withheld].

During examination of port lifeboat, PSCO found remote release wire dangling outside of lifeboat. Therefore, lifeboat was not ready for immediate use.
