The Company and the ship shall comply with the requirements of the ISM Code. The company should clearly define and document the master’s responsibility with respect to implementing the safety and environmental protection policy of the company. Indicated in the ship’s Ballast Water Management plan (chapter 14 pg. 19), the Master must ensure that the BMP is clearly understood by the appointed officer and by any other ship’s staff that may be involved. The company should establish procedures to ensure that new personnel and personnel transferred to new assignments related to the safety and protection of the environment are given proper familiarization with their duties. Indicated in the ship’s Ballast Water Management Plan (chapter 15 pg. 20), the officer in charge was to ensure the ballast water record book and any other necessary documentation/forms are completed and kept up-to-date. The Ballast Water Record book was not signed by the Master since 01SEP2022. A total of 08 completed pages were unsigned (pg 31-38). Last completed page was on 11NOV2022. Last records in the Ballast Water Handling Log was on 25SEP2022. Last records in the Ballast Water Handling Log was on 25SEP2022 for all ballast tanks. Officer in charge failed to maintain ballast water handling logs. Require an external audit related to the safety management procedures related to BWM within 03 month.

The Master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the U.S. must follow these practices: Rinse anchors and anchor chain when the anchor is retrieved to remove organisms and sediment at their place of origin. Starboard anchor was observed with mud from the previous port. Vessel shall not clean anchor while in U.S. waters unless actions will ensure organisms and sediment are contained.

The officers responsible to open the CO2 valves for #3 cargo hold failed to ensure fire systems was ready after ballasting operations.
