Repair or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of the deck. MI discovered the (02) aft paddle mount tabs were corroded and in need of repair. Vessel is permitted to continue operations but must have the deficiency rectified within 30 days. 46 CFR 176.700(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Repair/replace inoperable port side navigation light.

As appropriate for the intended voyage, a vessel must carry adequate and up to date U.S. Coast Pilot. Coast Pilot not on board.

A vessel not required to have a power driven fire pump must have at least three 9.5 liter (2 1/2 gallon) buckets. Replace the three 2 gallon buckets with three 2.5 gallons buckets.

Each battery used in an item of lifesaving equipment must be replaced during the annual inspection if the expiration date of the battery has passed. Replaced expired life buoy light battery.

Employers must maintain records of chemical tests, including pre-employment and random testing, and must make these records available to Coast Guard officials upon request. Owner had no record of pre-employment or random testing. Provide record of testing from ASAP to USCG. - 46CFR16.260(b)(1)

Marine employers may use sponsoring organizations, or may use contractors, to conduct the random chemical testing programs required by this part. Owner had no record of current year contract with consortium. Provide record of current year contract to USCG. - 46CFR16.230(d)

An EAP training program must be conducted for the employer’s crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; anddocumentation of training given to crewmembers and the employer’s supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Owner had no record of training for crewmembers or supervisor. Provide record of training to USCG. - 46CFR16.401(b)

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Owner had no alcohol test strips for testing within 2 hours of a serious marine incident. Provide. - 46CFR16.240

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Owner had no record of current year MIS report from consortium. Provide report to USCG. - 46CFR16.260(a)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of acommunity service hot-line telephone number for crewmember assistance, and display/distribution of employer’s policy regarding drug/alcohol use in workplace. Owner had no record of signed substance abuse policy for each employee as required by program. Provide record of signed policies for each employee to USCG. - 46CFR16.401(a)

Operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged for review by the Coast Guard upon request. The training entry shall include the following information: Date of the training and general description of the training topics.
