All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Observed damage to flexible hose on stbd engine transmission heat exchanger. Renew hose on stbd engine. 46 CFR 182.100

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Stbd engine electric submersible bilge hose found kinked unable to discharge properly. Bilge hose removed and replaced properly with new hose during inspection. 46 CFR 182.100

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Raw water air conditioner pump PEX fitting failed on discharge side of pump in starboard engine room creating a flooding situation. Fitting had slow drip leak as it was below the water line. Generator was started and AC was turned on exasperating the flooding of the starboard side engine room. Starboard engine room electrical submersible bilge pump was not operational and due to be replaced, and starboard engine room high level bilge alarm circuit breaker was switched into the off position preventing the alarm from sounding. Engine room was flooded with salt water above the engine block but below the top of the fuel tank top. Provide new engine, new electrical wiring, operational bilge pump, operational bilge high level alarm, and conduct sea trails with CG prior to departure. 46 CFR 182.100

All fiber reinforced plastic vessels constructed with general purpose resins must be fitted with a smoke activated fire detection system of an approved type installed in accordance with 76.27 in subchapter H of this chapter in accommodation spaces and service spaces. Vessel does not a have smoke activated fire detection system for required spaces. Provide smoke activated fire detection system and install to the satisfaction of the attending marine inspector. 46 CFR 177.410(c)(3)(ii)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's stbd fuel shutoff valve was not operable due to the way the pull cable is in line with the valve. Valve not fully closed so pull handle can operate. Provide operable pull handle shutdown. 46 CFR 182.455(b)(4)

At each inspection of a vessel, the owner or managing operator shall be prepared to conduct tests including the following: Inspection and operation of through hull fittings and sea valves. Starboard side engine raw water sea valve handle missing. No way to close valve. Provide means of closing sea valve. 46 CFR 176.802(a)(2)

All vessels must have operable navigation lights in accordance with the Navigation Rules. Stern light not showing full luminosity. Provide navigation lights that are in compliance with the Navigation Rules. 46 CFR 183.420

Each vessel must have permanent draft marks at each end of the vessel or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum trim and amidships to indicate the maximum allowable draft. No hull permanent markings on vessel. Provide permanent hull markings IAW stability letter. 46 CFR 185.602

All vessels must have navigation lights that are in compliance with Navigation Rules. Port light not operational. Provide operational Port nav light. 46 CFR 183.420

Electrical wiring must be installed with metal supports every 24 inches. Forward voids have wiring that was not supported. Provide support for wiring. 46 CFR 183.340

Machinery installed on a vessel must be suitable for the vessel and its operation and for the intended purpose. Starboard side engine fuel rack and piping heavily corroded. Replace fuel rack and piping with new equipment. 46 CFR 182.100

Vessel does not currently meet the requirements of Work Instruction 31. Mandated crew posted on COI may not enter the water or act as lifeguard until policy and procedure provided. Provide written procedure. Work Instruction 31

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

Hydrostatic release for EPIRB expired. Provide non-expired hydrostatic release for EPIRB.

First aid kit items expired. Review entire first aid kit, remove expired items and replace with new items.

No spacers between life floats. Provide a spacer to prevent life floats from sticking together.

01 Life float missing paddles. Provide 02 paddles for each life float.

Life float paddles did not have name of vessel marked. Provide name of vessel on each paddle in clearly legible letters.

Life float netting had hole. Make repairs to life float netting.

Ring buoy line and ring itself showing sever sun damage. Provide new ring buoy.

Starboard Forward high water bilge alarm visual and audible indicator at helm not operational. Provide visual and audible bilge indicator at helm.

Auto bilge visual indicators not operational. Provide light or other visual means at helm.

Port side engine fuel shut off could not fully close without first closing generator fuel shut off. Valve handles are in line causing engine closure to not close fully. Make change to fuel shut off to ensure both shut offs work without relying on the other shut off to be fully closed.

Port side engine flywheel guard missing. Provide guard for all rotating machinery.

Sail Preventative Maintenance Plan(PMP) not on vessel. Provide copy of PMP for vessel and provide electronic copy of PMP to [name withheld] for vessel files.

Drill and EPIRB logs not provided on vessel. Provide 12 months of past drill and EPRIB testing logs.

Navigation publication not on board vessel. Provide Coast Pilot, light list, tide tables, Nav rules.

Flares were expired. Provide new flares in accordance with 46CFR 176.808.

Lifebouy was deteriorated. Renew in accordance with 46 CFR 176.808.

Stbd engine compartment ventilation closure did not close completely. Repair/renew in accordance with 46 CFR 176.810.

Rigging Preventative Maintenance Plan was not provided. Provide Preventative Maintenance Plan required by 46 CFR 176.840
