EACH VESSEL VAPOR CONNECTION FLANGE FACE MUST HAVE A PERMANENT STUD PROJECTING OUTWARD THAT HAS A 12.7 MILLIMETER DIAMETER AND IS ATLEAST 25.4 MILLIMITERS LONG. IT MUST BE LOCATED AT THE TOP OF THE FLANGE FACE, MIDWAY BETWEEN BOLT HOLES AND IN LINE WITH THE BOLTHOLE PATTERN. PSCO OBSERVED THE STUD ON THE PORT SIDE MANIFOLD VAPOR CONNECTION SAWED OFF AND THEREFORE NO LONGER PERMANENTLY AFFIXED. 46CFR39.2001(j)

Oil tankers of 20000 tonnes deadweight and above, constructed on or after 01 January 2012, shall be provided with a fixed hydrocarbon gas dectection system complying with the FSS code for measuring hydrocarbon gas concentrations. The fixed hydrocarbon gas detection system referred to in chapter II-2 of the convention shall be designed, constructed and tested to the satisfaction of the the administration based on performance standards developed by the organization. The vessel failed to calibrate the fixed hydrocarbon gas detection system since December 2018. SOLOAS 14/II-2/4.5.7.3.1 FSS CODE CH. 16/2.1.1

Each person conducting the transfer shall stop the transfer operation whenever oil or hazardous material from any source is discharged in the transfer operation work area. Transfer operation was being conducted with a hydraulic powered booster pump at the starboard manifold that was leaking cargo out of the pump seals.

To discharge ballast water into the waters of the United States, the master, owner, operator, agent or person in charge of a vessel subject to 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard in 151.2030(a), use an AMS as described in 151.2025(a)(3), or ballast with water from a U.S. public water system, according to the schedule in paragraph (b) of this section. Subject vessel, of keel laid date 30OCT2015 and ballast water capacity of 8083 m3, is past its compliance date per the implementation schedule in 33 CFR 151.2035 (b). Vessel is fitted with BWTS but was not used and vessel intends to discharge / is actively discharging untreated ballast into U.S. waters. Vessel is fitted with BWTS but was not used to discharge approximately 6440 metric tons of untretaed ballast water while berthed at Texas International Terminals in Galveston Texas.

If the installed BWMS required by this subpart stops operating properly during a voyage, or the vessel's BWM method is unexpectedly unavailable, the person directing the movement of the vessel must ensure that the problem is reported to the nearest COTP or District Commander as soon as practicable. Vessel's BWTS became non-operational on 20SEP2017. Vessel had a port call in Houston Texas on 17NOV2017. Vessel's BWTS was not repaired until 13JAN2018. Vessel could not provide documentation to PSCO of reporting the non-operational BWTS to the Local Houston COTP.
