1160 - Holds and Tanks- The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. While de-ballasting the #6 cargo hold, the cargo tank ventilation was not open creating a vacuum in the tank. As a result, the # 6 cargo tank hatch cover was drawn completely into the tank and is resting at the bottom of the tank. Provide class report and repair plan prior to vessel movement.
