Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Identified inoperable port side jet engine. Determine root cause and conduct sea trials to the satisfaction of the attending Marine Inspector. 46 CFR 119.200(a)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 or be specifically approved by the Commandant. Fire main found with non compliant hose on 21NOV25. Two sections were verified satisfactory on 26NOV25, but entire length is not fully compliant. Install SAE 1942 compliant hose in entire length of non metallic portions of fire main. 46 CFR 182.720

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 or be specifically approved by the Commandant. Flexible nonmetallic hose installed in fire main in machinery space does not meet SAE J-1942. 46 CFR 182.720

At each inspection for certification and every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Starboard side rail and bulwark damage identified on 29JUL25 has not been properly repaired. On 6NOV2025, Marine Inspectors identified new damage along the starboard side that requires permanent repair. Provide repair proposal for all starboard side damage prior to affecting repairs and verify satisfactory repairs through the local OCMI. 46 CFR 176.830

At a minimum, each placard must notify the reader of the operating requirements contained in 151.67 through 151.73 as they apply to that ship. The following requirements should also be prominently stated: the discharge of all garbage is prohibited into the navigable waters of the U.S. and into all other waters except as specifically allowed; discharge of all forms of plastic into all waters is prohibited; a person who violates the above is liable for civil and/or criminal penalties; and regional, state, and local restrictions on garbage discharges also may apply. Observed that the garbage placard was missing. (33 CFR 151.59 (c))

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked "First-Aid Kit". A first-aid kit must be easily visible and readily available to the crew. Observed that first aid kit had expired materials. (46 CFR 184.710)

Whenever damage or deterioration to hull plating or structural members may affect the seaworthiness or a vessel is discovered, the cognizant OCMI may conduct an internal structural examination in any affected space and may require the vessel to effect permanent repairs. MI observed 1/16" pitting in the vessel's STBD void in passenger cabin. Submit repair proposal and affect repairs to the satisfaction of the MI prior to next credit dry dock (46 CFR 176.600(d))

Vessels gangway handrails broke in 2 areas at the weld seams. Conduct repairs to the handrails to the satisfaction of the MI.

MI observed 12" fracture on top of the starboard bulwark located outboard of main deck fire main and 3" fracture to the outboard bulwark below. Provide repair proposal and affect permanent repairs to the satisfaction of MI. 46 CFR 176.830

MI observed crack along base of starboard aft rail to bulwark connection located just aft of loading door. Provide repair proposal and affect permanent repairs to the satisfaction of MI. 46 CFR 176.830

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personal injury. MI observed outboard side of raw water cooling pipe for starboard main engine leaking a constant stream of water from a pinhole fracture. 46 CFR 182.100

Each rescue boat and ring life buoy must be marked with Type II retroreflective material IAW 46 CFR 164.018 or other standard specified by the Commandant. MI observed ring buoy without Type II retroreflective material. 46 CFR 185.604(i)

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. MI observed house battery compartment on starboard side with significant water ingress from splash and spray due to failed hatch gasket. 46 CFR 183.350(b)

A vessel with batteries of adequate capacity to supply the loads specified in paragraph (a)(1) of this section for three hours. MI observed 12V house battery arrangeement lose approximately 2V of charge without load in a 15 minute period. Port side battery is unable to hold charge without shore power, or alternator for required time. 46 CFR 183.310(a)(2)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas system. Quick closing ventilation dampers are not closing fully without assistance from crew in spaces. 46 CFR 182.460(h)

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Steering shaft at the top of jet drive on starboard side leaks approximately one tablespoon when put to port or starboard. Conducted repairs to satisfaction of attending Marine Inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Starboard outboard structural support that connects from transom to beneath swim platform is fractured in three locations. Submit repair proposal to crop and renew fractured structure.

Whenever there is a hazardous condition, as defined by 46 CFR 175.400, on board the vessel, the owner, master, agent, or person in charge shall immediately notify the Captain of the Port of the port of place of destination and the Captain of the Port or place in which the vessel is located of the hazardous conditions. Marine Inspector observed active water leak from starboard jet drive steering lever which onboard records indicate was leaking at an average rate of 12 gallons per day and has been occurring for more than 30 days. Affect proper repairs to stop water ingress and conduct satisfactory sea trials with a Marine Inspector present.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. During inspection of starboard main engine, Marine Inspector found pockets of collected engine oil on top of the engine, oil soaked lagging, and oil saturated sorbent materials stuffed in several areas of the engine to collect an ongoing oil leak. Determine source of leak, provide corrective actions and conduct operational test with a Marine Inspector present.

At each inspection for certification and at every other vessel inspection, the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard or clog bilge pumping systems. Marine Inspector found several gallons of oily water and multiple oil soaked sorbent pads floating in the bilge due to ongoing water ingress and starboard engine leak. Properly remove all oil, water, and saturated materials from the starboard engine compartment and complete reinspection with local OCMI.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10; for fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. Marine Inspector found all fixed and portable fire suppression systems onboard overdue for servicing since 31JUL24. Review of onboard records showed crew noted overdue fire fighting systems and management failed to take appropriate action to rectify. Provide evidence of servicing for all fire fighting equipment onboard to the satisfaction of attending Marine Inspector.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the [name withheld] observed five spots with significant pitting on the fire main piping approximately three feet above the main deck that were covered over with temporary repairs that require crop/renewal of pipe.

...(a) Each EPIRB, must be tested monthly. (b) The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner; and (c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable... EPIRB battery is expired, and missing monthly testing from January 2023- June 2023, replace battery and ensure monthly testing. 46 CFR 185.728(a)(b)(c)

A person may not employ or engage an individual in a position required to hold an MMC unless that Individual maintains a current medical certificate. Provide a current medical certificate. 46 CFR 15.401(c)

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections Provide marine inspector with servicing report of fire equipment. 46 CFR 176.810(b)(iv)

At least one ring life buoy must be fitted with a lifeline. At least one must not have a lifeline attached. Each lifeline on a ring life buoy must: (1) Be buoyant; (2) Be of at least 18.3 meters (60 feet) in length; (3) Be non-kinking; (4) Have a diameter of at least 7.9 millimeters (5/16 inch); (5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and (6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light... Life ring on main deck is deteriorated replace life ring and lifeline. 46 CFR 180.70

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with roll and pitch of the vessel, and free from exposure to water splash or spray... Starboard engine room inboard battery has a cracked lid.... Repair or replace battery box lid. 46 CFR 183.350(b)

A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals and (2) Three hand orange smoke distress signals. Vessel is missing one red hand flare. 46 CFR 180.68(b)(1)&(2)

Machinery space boundaries. Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant...Wire transits in Port and Starboard engine room vapor barrier have a foam/plastic wrapping, remove wrapping from wires transiting the vapor barrier. 46 CFR 177.410(c)(4)

All equipment, including switches, fuses, lampholders, etc., must be suitable for the voltage and current utilized... Fuse on Starboard wiring panel is severely corroded, replace fuse and clean the area. 46 CFR 183.220(b)

A suitable hull strainer must be installed in the circulating raw water intake line of an engine cooling water system.... Clean or replace sea strainer on Port and Starboard main engine raw water intake. 46 CFR 183.430

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request... Provide marine inspector with chemical testing records to complete DAPI audit. 46 CFR 16.260(a)

The initial inspection may include an inspection of the following: The arrangement, installation... including piping, main and auxiliary machinery... pollution prevention equipment. Port and starboard engine room bilges have excessive oil and debris. Engine room bilges were not tested due to this. Clear engine room bilges and ensure no oily residue remains in the lines. 46 CFR 176.402(c)(1)

An inspection for renewal of a certificate of Inspection is conducted to determine if the vessel is in satisfactory condition...the OCMI may require the vessel to get underway. Underway portion of COI inspection not completed. Request subsequent inspection to witness man overboard drills and underway checks. 46 CFR 176.404(a)

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: (iv) Navigation equipment and lights.... Vessels navigation equipment inoperable. Make repair to the satisfaction of the marine inspector. 46 CFR 183.310(a)(1)(iv)

A vessel must comply with the applicable requirements for any radio installations as set forth by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessels radio equipment is inoperable, make repair to the satisfaction of the marine inspector. 46 CFR 184.502

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Strainer was not three times the diameter of the bilge pipe. 46 CFR 182.510(b)

Electrical equipment on a vessel must be installed and maintained to: protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in the electrical equipment, and electrical shock. Wires in port forward space under deck were not properly secured or locked out from breaker panel. 46 CFR 183.200(b)

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The name of the vessel was not marked on the port or starboard bow. 46 CFR 67.1239(a)

Piping, including valves, pipe fittings and flanges, conveying vapor, gas or liquid, the temperature of which exceeds 65.5C, must be suitably insulated where necessary to prevent injuries. Exhaust lagging on both engines was worn and needs to be replaced. 46 CFR 177.970

There must be a means of accurately determining the amount of fuel in each fuel tank either by sounding, through a separate sounding pip or a fill pipe, or by an installed marine type fuel gauge. 46 CFR 182.445(b)

Each fire drill must include: Instruction in the use and location of fire alarms, extinguishers, and any other fire fighting equipment onboard. Vessel needs instructions on board indicating that the ignition must be turned in order to use FIREBOY override function.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Vessel is missing a chart, provide proof of updated charts to marine inspector.

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. Firemain hose is currently shield sanitation hose and is not to be used for the firemain. Replace with SAE J-1942 or equivalent hose.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Bilge system hose currently single clamped.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Strainers missing from bilge piping in both the port and stbd aft sections of the engine spaces.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Flywheels are exposed in both engine spaces. Provide a method of protection from hazard.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. COD Application has been submitted to NVDC. Provide copy of COD once received.

The provisions of Part III of Title III of the Communication Act require United States vessels which transport more than six passengers for hire while such vessels are being navigated on any tidewater within the jurisdiction of the United States adjacent or contiguous to the open sea, or in the open sea to carry a radiotelephone installation complying with this subpart. Vessel is missing the radiotelephony safety certificate. 47 CFR 80.901
