The fire detection and alarm system must be capable of immediate operation at all times that the vessel is in service. MI witnessed an active fault for the alarm panel power system. Repair/replace to the satisfaction of the the Marine Inspector and provide report from maintenance or a fire servicing technician. Cite: 46 CFR 181.450(b) 46 CFR 76.27-25

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection and testing of all smoke and fire detection systems, including sensors and alarms. 46 CFR 176.810(a)(7)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Vessel Master was unable to provide proof of marine radio operator permit. Provide evidence of marine radio operator permit to MI. 47 CFR 80.159(e)

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The records of crew training and drills were not available during inspection. Provide evidence of training and drills to MI. 46 CFR 185.420(d)

Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who:(1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection;(2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or,(3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. Evidence of pre-employment/random testing performed by third party were not available. Provide evidence of drug program enrollment and testing to MI. 46 CFR 16.230(a)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Pressure gauge on #1 ballast pump piping was significantly damaged. replace gauge to satisfaction of MI. 46 CFR 182.100

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. The sea chest strainer is leaking. Repair or replace to the satisfaction of the attending marine inspector. - 46 CFR 182.700

Each ring life buoy on a vessel must: Be approved in accordance with 160.050 in subchapter Q of this chapter, or other standard specific by the Commandant. The #1 direction port side ring life buoy is missing a plug and the filler material has degraded. Replace to the satisfaction of the attending marine inspector. - 46 CFR 180.70(b)(1)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel lost propulsion and power after exiting the slip. Rectify Prior to Movement and Prior to the Carriage of Passengers to the satisfaction of the USCG Marine Inspector. 46 CFR 182.200(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel lost generator, which powers the motor for propulsion. Determine cause and repair to the satisfaction of the attending Marine Inspector.

Vsl lost propulsion after material failure of the control card resulting in the vessel alliding with the ferry ramp.

Vessel reported a loss of propulsion on their Island side main propulsion engine. Vessel will troubleshoot and repair the engine and preform operational tests to the satisfaction of the attending marine inspector.

Propulsion machinery must be capable of operating at constant marine load. While underway the vessel reported losing all propulsion and steering due to a breaker trip on the T1 distribution panel. Prior to carrying passengers/cargo vessel must demonstrate to the satisfaction of the cognizant OCMI that it is able to meet all operational requirements.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] the vessel began to pull away from the dock, the #1 and #3 generators kicked off line resulting in a complete blackout. The #2 generator was reset and recovered partial power back to the vessel allowing a safe return to the dock. Technicians will troubleshoot and provide a report once the issue has been resolved.
