Vessel's rescue platform does not meet the specific guidelines as per Sector St. Petersburg's Work Instruction MPS-WI-SECSP, Rescue Platforms as an Alternative to Rescue Boats on Small Passenger Vessels. Specifically, MI's observed that the rescue platform had cracked and incomplete welds making it not serviceable. 46 CFR 180.210(b)(2)

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible. There was no COI posted on the vessel. 46 CFR 176.302

The stability letter must be poster under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. There was no stability letter posted on the vessel. 46 CFR 176.306

Vessel only has 6 red hand flares onboard. Provide an additional 6 red hand distress signals or 6 orange hand smoke signals. 46 CFR 180.68(a)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following compartments: A space with a through-hull fitting below the waterline. Steering space bilge high level alarm inoperative. 46 CFR 182.530(a)(1)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of the individual who was directly involved in the SMI. Alcohol testing swabs on board were expired and vessel Capt must store in an area that is within temperature rating as noted on swab package. 46 CFR 4.06-15(a)(1)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers. Current employee list not provided. 46 CFR 16.230

The master and crew of a vessel will be familiar with the content of and and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions are not posted. 46 CFR 185.510(a)

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: the vessel carries passengers on an open or partially enclosed deck and the cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. The rescue platform at the transom does not conform to the OCMI specifications and does not meet the following: The platform landing shall be a minimum of 36 x 36 inches. The platform landing shall be constructed of expanded metal (mesh) to reduce the effect of wave action. The platform, when deployed, must be parallel to the water without breaking the surface. The platform must be of substantial construction and should be capable of supporting at least two persons (370 lbs) and must be able to withstand the effects of wave action. 46 CFR 180.210

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Anchor light was not operational when tested. 46 CFR 183.420

All electrical equipment and circuits must be marked and identified. AC breaker from shore power to battery charger is not labeled and missing dead front cover. 46 CFR 183.220(d)

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. The fire-boy system shut down the main engine as required, however when the override switch was activated the engine was only able to start after the crew manually switched the battery selector to battery bank #2. This is not as designed by systems operation manual. 46 CFR 181.420(b)(3)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. There was no life jacket donning placard located on board the vessel. 46 CFR 185.516(a)

A pre-engineered fixed gas fire extinguishing system must: Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector. Remove maintenance pin preventing manual actuation. Corrected on the spot. 46CFR181.420

An EPIRB must be installed to automatically float free. Replace expired Hydro-static release. 46CFR180.64

Each ring life buoy must be marked with Type II retro-reflective material approved in accordance with 46 CFR 164.018. Replace worn ring buoy reflective material. 46CFR185.604

A vessel must carry either a first-aid kit approved under 160.041 or a kit with equivalent contents. Replace existing kit with above standard. 46CFR184.710
