In tankers, electrical equipment, cables and wiring shall not be installed in hazardous location unless it conforms with standards acceptable to the organization. Multiple connections on all cargo pumps have either back shell gasket not properly sealed or wire pulled from back shell not allowing for back shell gasket to make contact.

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 mm (.5mm) diameter and is at least 25.4 mm (1 in) long. It must be located at the top of the flange face, midway between bolt holes and in line with the bolt hole pattern. Vessels vapor stud was removed due to the flange design on the facility equipment not lining up. All four vapor studs on vessel are not permanent and can be removed.
