The company and the ship shall comply with the requirements of the International Safety Management Code. The technical nature of deficiencies 02, 03, and 04 provide objective evidence of a serious failure, or lack of effectiveness, of the implementation of the ISM Code. Additionally, the SMS A safety management audit carried out by the Administration or the RO, with a corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of this deficiency and release from detention. 74 SOLAS (24) IX/3.1 / ISM Code Part A/10.1

The ship's load line shall not be submerged at any time when the ship puts out to sea, during the voyage, or on arrival. The primary load line reference was found submerged by approximately 10-15 cm. Review of the departure drafts, fuel burn rate, current trim, and crew statements confirmed that the load line was submerged during transit. Correct the overloaded condition and review current procedures and loading calculations to ensure load line limits are continuously maintained and not exceeded during future cargo operations. 66 ILLC (05) Article 12.1

The venting arrangements shall be so designed and operated as to ensure that neither pressure nor vacuum in cargo tanks shall exceed design parameters and be such as to provide for the passage of large volumes of vapor, air or inert gas mixtures during cargo loading and ballasting, or during discharging. Vessel is unable to monitor pressure or vacuum pressure during cargo operations due to faulty cargo monitoring equipment. 74 SOLAS (04) II-2/11.6.3.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel's installed fixed gas detection system was unable to provide means of operational testing. 74 SOLAS (24) I/11

The master shall be supplied with such information to the satisfaction of the Administration as is necessary to enable him by rapid and simple processes to obtain accurate guidance as to the stability of the ship under varying conditions of service. A copy of the stability information shall be furnished to the Administration. PSCO observed loading computer displaying inaccurate ballast and stability information with a difference of up to 10 metric tons when comparing lightship characteristics from the computer-based Kockum Sonics AB Loading Control System 6374.090917, Version 5.0 to the approved Trim and Stability Booklet Dwg. No. 101-350, approved by DNV on July 13, 2024. The vessel's loading computer for trim and stability should be updated to provide accurate information to determine vessel's loading specifications. 74 SOLAS (04) II-1/5.1.1

For tankers of 20,000 tonnes deadweight and upwards constructed on or after 1 July 2002 but before 1 January 2016, the protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety Systems Code, as adopted by resolution MSC.98(73), except that the Administration may accept other equivalent systems or arrangements, as described in paragraph 5.5.4. PSCO observed the vessel's inert gas system nonoperational not available for testing to verify operation and suitability. 74 SOLAS (04) II-2/4.5.5.1.1
