NATIONAL GEOGRAPHIC QUEST

Whenever a vessel is deprived of the service of a member of its complement, and the Master or Person in Charge is unable to find appropriate credentialed personnel to man the vessel, the Master or Person in Charge may proceed on the voyage, having determined the vessel is sufficiently manned for the voyage. Vessel is authorized to sail short 01 C/E until arrival in Colon, Panama on 23JAN2026.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported non-operational stbd engine. Determine cause and complete repairs to satisfaction of Marine Inspector. Vessel code amendment (60 to 16): Vessel was able to reset ECU and start engine. Provide remote survey technician report completed on 26MAY2025, and tech report of attending technician within 14 days.

If a survival craft is recovered by electric power, the electrical installation, including the electric power-operated boat winch, must meet the requirements in sub-chapter J of this chapter. If a survival craft is recovered by any means using power, including a portable power source,safety devices must be provided that automatically cut off the power before the davit arms or falls reach the stops in order to avoid over-stressing the falls or davits, unless the motor is designed to prevent such over-stressing. MI witnessed a failed test of both davit limit switches.

A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. #1 SSDG was down for corrective maintenance due to generator not producing the proper voltage.

Maintenance, testing and inspections shall be carried out based on the guidelines developed by the Organization* and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. MI observed 02 faults on the water mist control panel. The two faults were - system fault and pump unit fault. Prior to departing, faults shall be cleared and system tested to ensure full operation.

Vessels described by 46 CFR 114.110(f) must submit plans to Marine Safety Center and cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. 29 April 2024, Sector San Diego issued an 835 for the vessel not meeting the regulation for Watch Monitoring Device. MI observed that vessel has not installed an approved Watch Monitoring Device. Prior to carrying passengers, vessel shall install an approved Watch Monitoring System.

Vessel is not adequately equipped with roving watchman alarms. Prove compliance with 46 CFR 122.410(b), or submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring devices to ensure the wakefulness of the vessel's roving watchmen. The watchman alarms must meet the requirements in 46 CFR 122.410(b). Deficiency to be rectified by 29 May 2024. Cites: 46 CFR 122.410(b) and 46 CFR 114.110(f).

Structural fire protection in the following spaces was found to be damaged: Starboard side HVAC (upper deck), Bosun Locker, Grease Trap Space, Portside HVAC (main deck). Repair the damaged structural fire protection to the satisfaction of the attending USCG marine inspector. Deficiency is to be rectified by 06 May 2025 or else this deficiency becomes "Action: 17 - Rectify prior to departure". Cite: 46 CFR 116.415 and SOLAS 74 II-2:9.2.2

MI found temp repairs on both ship service generator prime movers lube oil pump drain lines. Provide evidence of permanent repairs before embarking passengers and departing from the first U.S. port.

MI found both port and starboard rescue boat davit limit switches not working as designed. Provide evidence of permanent repairs before embarking passengers and departure from the first U.S. port.

MI found port and starboard rescue boat davit accumulators not holding pressure as designed. Provide evidence of permanent repairs before embarking passengers and departure from first U.S. port. Crew must conduct daily checks on both davits to ensure system has proper pressure to launch rescue boats as designed.

MI found all relief valves for the ship service pressure vessel not serviced as required by the regulation. Provide evidence of servicing of all relief valves prior to departure of next U.S. port.

MI found automatic closing door 413A (AFT Galley Door) not closing as designed. Door shall be closed when not in use. Provide evidence of permanent repairs before embarking passingers and departure from the first U.S. port.

MI found trapped personnel alarm for the freezer not working as designed. Provisde evidence of permanent repairs.

Captain reported four 100 person liferafts were not able to be serviced in time. Three 35 person rafts were placed onboard in addition to two 50 person rafts and three 25 person rafts bringing total capacity to 280. 135 persons on port side, 145 persons on starboard. Vessel's New Construction SOLAS Lifeboat Exemption Request dated 13MAY2016, requires for lifesaving equipment to provide an aggregate capacity for 336% of the total number persons onboard requested at 149 total persons. Considering life raft capacity of 135 persons port side and 145 persons starboard side, the maximum number of persons allowed on board while on an international voyage is constrained to 80 persons. SOLAS 74 (14)III/21.1.1.1 -16C

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Fuel shutoffs valves for the following systems were seized and not operational; 2 boilers, 2 main engines, 2 generators. Demonstrate proper operation of shutoff valves to the satisfaction of attending marine inspector. 74 SOLAS (14) I/11

General arrangement plans shall be permanently exhibited for the guidance of the ship’s officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and fire alarm systems. Plans and booklets shall be kept up to date. Current version of posted fire control plan is not approved. 74 SOLAS (14) II-2/15.2.4.1

No ship to which the present Convention applies shall proceed to sea on an international voyage after the date on which the present Convention comes into force unless it has been surveyed, marked and provided with an International Load Line Certificate (ILLC). ILLC is expired. Provide valid ILLC prior to sailing international. ICCL 1988 Annex A Article 3

Ducts with a free cross-sectional area exceeding 0.075 m2 shall be fitted with a fire damper. The switch labeled "Dishwasher Exhaust Damper" is actually the upper end galley hood damper. Provide evidence the dishwasher exhaust is fitted with a damper or is below the size requiring one. - 74 SOLAS (14) II-2/9.7.3.1.2

The Continuous Synopsis Record shall list the name of all classification societies with which the ship is classed. The vessel has a classification certificate from Bureau Veritas, but this is not reflected on the CSR. Provide updated CSR. - 74 SOLAS (14) XI-1/5.3

The fire damper identification shall placed on any controls required. Fire damper controls in galley and cleaning gear room are mislabeled or unlabeled. Provide proper labeling of all damper controls. - 74 SOLAS (14) II-2/9.7.3.1.2

On passenger ships constructed on or after 1 July 2008, non-load-bearing partial bulkheads which separate adjacent cabin balconies shall be capable of being opened by the crew from each side for the purpose of firefighting. Balcony partitions are bolted into place. Provide means for opening by fire teams.

Watertight doors 1,2, and 3 failed to operated under stored power. Prove proper operation of watertight doors. - 74 SOLAS (14) II-1/13.7.3.3

Port side liferaft painter not connected to weak link. Properly attach liferaft for float-free arrangement. - 74 SOLAS (14) III/14.4.1

Combustible storage is not permitted in category 10 electrical lockers. Remove items stored in electrical locker off main dining room port side forward. - 74 SOLAS II-2/9.1.2

The gas detection equipment employed for confined space entry and rescue is out of calibration. Calibrate gas detection equipment. - 74 SOLAS (14) III/19.3.6.2.3

Storage cabinets are not permissable furniture in stairway enclosures. Remove storage locker at the bottom landing of main stairwell. - 74 SOLAS (14) II-2/5.3.3

Penetrations in fire-resistant decks/bulkheads must be of equivalent fire rating. Remove rubber air hoses leading out of the engine room on the starboard side aft that penertrate through fire stops and restore fire rating of decks/bulkheads with approved fire-resistant materials. - 74 SOLAS (14) II-2/

First aid kits onboard missing equipment required under 46 CFR 160.041. Provide first aid kit that contains all required equipment and replaced expired aspirin in conference room kit. - 46 CFR 121.710

No emergency lighting illuminates the landing at the top of the ladder for the engine room emergency escape. Provide emergency lighting at landing during next drydock. - 74 SOLAS (14) II-1/42.4.1.1

Insulation on fire boundaries throughout the vessel damaged or missing, including in engine room, elevator machinery space, forward crew stair, and dining room electrical locker. Repair or replace damaged/missing insulation at next drydock.

Person in charge of passenger mustering on station bill (Senior Steward A) missing evidence of completion of Crisis Management and Human Behavior training. Provide evidence of training completion. - STCW 95 V/2

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel expierenced loss of propulsion from PORT main engine. Provide technician report proving satisifactory repairs and operation of PORT main engine. 46CFR115.840

AIS must be maintained in effective operating condition, which includes the accurate input and upkeep of all AIS data fields. The vessel is transmitting inaccurate AIS data. 33 CFR 164.46(d)(2).

Fire dampers must be accessible from for periodic inspection by means of a hinged or bolted plate in the duct and surrounding bulkhead/deck. Dampers have no inspection ports. Provide ports for inspections of dampers. - 46 CFR 116.(g)(2)/74 SOLAS (14) II-2/9.7.4.6

For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as shown in categories (1) to (14). Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. Rooms classified as (10) "mechanical rooms" have been converted into storerooms without the required (13) arrangements or MSC approval. Remove all storage in mechanical rooms or install proper level of fire protection with MSC plan approval. 74 SOLAS (14) II-2/9.2.2.3.2.2

Furniture in stairway enclosures shall be limited to seating. Lockers of non-combustible material, providing storage for non-hazardous safety equipment required by these regulations, may be permitted. A combustible cabinet full of paper products and a metal locker not labelled to contain safety equipment are present in the aft stair. Remove prohibited furnishings from stairway. - 74 SOLAS (14) II-2/5.3.3

The transitional source of electrical power shall power emergency lighting at every embarkation station and over the sides for 30 minutes. The emergency lights for the embarkation station and over the sides are not supplied by transitional power. 74 SOLAS 14 Ch II-1 42.4.1.1

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage or day tank above the double bottom must be fitted with a remote actuated closing device. The remote closing device for the forward fuel tanks is not functional. 74 SOLAS 14 Ch II-2 2.2.3.4

Emergency lighting for mustering and abandonment shall be tested at each abandon ship drill. Emergency lighting is not tested during each abandon ship drill. 74 SOLAS 14 Ch III 19.3.4.9

Every passenger ship shall be provided with means for two-way onscene radiocommunications for search and rescue purposes using the aeronautical frequencies 121.5 MHz and 123.1 MHz from the position from which the ship is normally navigated. The required means are not provided. Provide suitable radios to communicate with aircraft on 121.5 MHz and 123.1 MHz. - 74 SOLAS (14) IV/7.2.

All ships of 500 gross tonnage and upwards shall have a gyro compass bearing repeater, or other means, to take bearings, over an arc of the horizon of 360º, using the gyro compass or other means referred to in subparagraph .1. The current arrangement of the alidade provides a ~120 degree blind sector due to mast and inflatables. Arrange gyro bearing repeater to permit the taking of bearings over a 360 degree arc. - 74 SOLAS (14) V/19.2.5.3

Make port rudder available for inspection at next scheduled dry dock, not later than 31OCT18.

Complete remainder of MSC approved alterations for escape arrangements, specifically installing break out windows prior to 07NOV18.

EH01: Vessel requires tug assist with tugs of adequate size, horsepower, and bollard pull for all movements within the Sector Columbia River Area of Responsibility.

EH02: While passengers are onboard additional engineering/safety rounds must be conducted in the aft steering space and engine room.

EH03: Vessel may not transit with passengers until a dry dock inspection has been completed and satisfactory repairs have been completed and witnessed by USCG.

EH04: Submit repair proposal to USCG for damages sustained during grounding.

Provide approved PSTP, vessel currently does not have approved PSTP.

Submit revised plans to MSC with compliant escape arrangements as proposed by your ltr dated 29DEC17.
