It has been self reported that on 14 February 2026 at 0530 local time the main engine received an auto slow down due to a faulty No. 5 FWD cylinder fuel injector. It is alleged that the injector's cut off shaft became clogged due to degradation of the associated discs underneath the injector spring.

Vessel experienced Oil Mist Detection (OMD) alarm triggered an emergency shut down signal to #2 SSDG, blacking out the plant with temporary loss of propulsion. the power management system failed to bring the standby generator on automatically.

Vessel experienced a restriction of the primary steering system while conducting pre-departure steering checks due to a potential faulty component in the feedback signal loop while operating the #2 hydraulic steering pump at the conning position, reducing the maneuverability of the vessel. Vessel to provide class report attesting to the proper repairs and operation of the steering system at all locations of operation.

or persons on board. Vessel experienced multiple marine
casualties in a short period of time, a loss of
propulsion on the 14th of November due to failed fuel
injector on the Main Propulsion Engine #4 cylinder and
on the 17th of November the vessel came to all stop with
a high-pressure fuel oil leak due to a failed “O” ring.
Vessel to provide class report attesting to the proper
repairs on the high pressure fuel oil system, proper
repair for the #4 cylinder Main Engine injector and the
proper operation of the Main Propulsion Engine.
All above items are to be completed to the satisfaction
of the CG Marine Inspector.

Subject to regulation 3 of this Annex, in an Emission Control Area designated for Tier III NOx control under paragraph 6 of this regulation, the operation of a marine diesel engine that is installed on a ship: is prohibited except when the emission of nitrogen oxides from the engine is within the following limits. Vessels main engine exhaust gas recirculation system has a cooling water leak and not able to operate in Tier III compliance.
Vessel is authorized to proceed directly to the next port of call in Oakland, CA to complete repairs to the exhaust gas recirculation system subject to any additional conditions of operation issued by ABS.
