Repairs or alterations to the hull that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel rep reported during a regular routine inspection of the vessel, the starboard side shell was found damaged with a breach present at the frame 5.0 stiffener. The lower rub rail was also found cracked along the weld seam forward of the breach at frame 5.0-6.0. The interior stiffener is bent/cracked at the point of instrusion. A minor bend of the stiffener is also present below the intrusion point near where the rub rail sits externally. All damage was reported to being localized above the water line. Make permanent repairs in accordance with submitted plan to CG Marine Inspector. Keep CG Marine Inspector informed of required; fit-up, back-gouge, and NDT of repair stages.

DUE TO THE EXTENSIVE CORROSION IDENTIFIED ON THE H100 SERIES VESSELS, THE VESSEL'S HULL MUST BE MADE AVAILABLE FOR INSPECTION BY A COAST GUARD MARINE INSPECTOR. PRIOR TO STARTING ANY WORK, A REPAIR PROPOSAL SHALL BE SUBMITTED TO THE SECNY OFFICE FOR REVIEW AND APPROVAL. ALL REPAIRS MUST BE CONDUCTED IN ACCORDANCE WITH THE APPROVED REPAIR PLAN, AND INSPECTED BY THE ATTENDING MARINE INSPECTOR.
