At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to have the vessel ready for inspection of lifesaving equipment and systems, including the inspection of each hydrostatic release unit...The vessel's hydrostatic release for the inflatable buoyant apparatus was incorrectly arranged, operator made correction on the spot. 46 CFR 176.808(a)(5)

All fiber reinforced plastic (FRP) vessels constructed with general purpose resins must be fitted with a smoke activated fire detection system of an approved type, installed in accordance with § 76.27 in subchapter H of this chapter, in accommodation spaces; service spaces; and isolated spaces that contain an ignition source. Vessel did not have a smoke detector installed in engine room. 46 CFR 177.410 (c)(3)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Identified areas beneath exterior benches that were connected to engine room (ER) and allowed air to flow freely. Provide appropriate means of sealing ER space to the satisfaction of the MI. 46 CFR 176.830

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the inspection and operation of all weathertight closures above the weather deck and the provisions for drainage of sea water from the exposed decks. Hatches for lazarette and engine room did not prevent water intrusion. Conduct repairs to create watertight integrity of the two hatches to the Satisfaction of the MI. 46 CFR 176.802

A pre-engineered fixed gas fire extinguishing system must: (2) Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector. Vessel’s fire boy system was found with the maintenance safety pin still install after servicing, causing the system to be unable to use its manual activation. Remove safety pin to the satisfaction of the Marine Inspector. 46 CFR 181.420(a)(2)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and(2) General description of the drill scenario and training topics. Vessel was unable to provide the general descriptions of the Abandon ship and man overboard drills as required. Fill out the drill conducted during the inspection to the satisfaction of the Marine Inspector. 46 CFR 185.520(f)

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Vessel has no records of crew training. Conduct training and properly document the training to the satisfaction of the Marine Inspector. 46 CFR 185.420(d)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel did not have alcohol testing devices on board. Procure new alcohol testing devices. 46 CFR 4.06-15(a)(1)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel was unable to provide their IBA servicing report. Supply Marine Inspector with proper servicing report. 46 CFR 185.702(d)

The master or person in charge of a ship, including a drilling rig or platform, to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. These locations must be readily accessible to the intended reader and may include embarkation points, food service facilities, garbage handling spaces, living spaces, and common areas on deck. Vessel did not have a placard that met the regulations of this part on board. Vessel owner must procure a placard and install on the vessel to the satisfaction of the Marine Inspector. 33 CFR 151.59(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced knocking on port engine, leading to vessel being hauled out for repairs. Provide root cause and conduct repairs to the satisfaction of the Marine Inspector. 46 CFR 182.200 (a)

Repairs or alterations to the hull, machinery, or equipment that effect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel is conducting repairs to the main engine without notifying the OCMI. 46 CFR 176.700

If the annual inspection reveals deficiencies in your vessel's maintenance, you must make any and all repairs or improvements within the time period specified by the OCMI. Vessel was issued ten deficiencies on 10JUN2024 that were to be rectified within 14 days, without correcting any as of 17JUL2024. Vessel is to rectify all deficiencies, referenced on the 835V issued 10JUN2024, within 14 days of issuance of this new 835V. 46 CFR 176.500 (1)(iii)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel unable to provide required documentation for drug and alcohol program. Provide required documentation to satisfaction of MI within 7 days in order to complete USCG chemical testing program compliance audit. 46 CFR 185.100

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel's drug and alcohol program is not in compliance with 46 CFR Parts 4 and 16, 49 CFR 40, and 33 CFR 95. Vessel must provide proof of pre-employment testing, company program, training, and any additional required documentation to demonstrate fully compliant program prior to the carriage of passengers. 46 CFR 185.100

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel has not completed drydock examination in the past two years. Conduct satisfactory drydock examination prior to the carriage of passengers. 46 CFR 176.600(c)(1)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port engine is inoperable. Repair engine to the satisfaction of the marine inspector. 46 CFR 182.200(a)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. STBD fuel shutoff valve is wasted and inoperable from reach-rod without breaking apart. Replace fuel shutoff valve assembly and complete operational fuel shutoff test to satisfaction of marine inspector. 46 CFR 182.455 (b) (4)

Portable and semi-portable extinguishers must be inspected and maintaned in accordance with NFPA 10: (iv) The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. 10 bc portable fire extinguisher at upper operating station requires annual servicing. Provide satisfactory evidence of required annual maintenance and servicing to satisfaction of Marine Inspector. 46 CFR 176.810 (b) (1)

Vessels shall be inspected for compliance with the standards required by this subchapter. For items not covered by standards in this subchapter, inspection shall be in accordance with the standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. Port Side fuel fill fitting on deck has a wasted gasket, allowing water intrusion to mildly saturate deck. Deck is showing some small signs of furring. Replace gasket to ensure satisfactory seal between deck fitting and deck at Port Side Fuel Fill. 46 CFR 176.25-1

Electrical equipment on a vessel must be installed and maintaned to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and shock. Exposed dead end wires visible at upper operating station. Remove dead-end wires station to satisfaction of Marine Inspector. 46 CFR 183.200 (b)

All machinery and equipment must be and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Hazardous condition exists in engine room from excessive gases present from "blow-by" system. Vessel shall either repair engine or provide technician/diagnostics report from a qualified Marine Diesel Technician of safe and effective operation of both Main Diesel Engines to the satisfaction of Marine Inspector. 46 CFR 182.100

Flexible hose runs shall be visible, easily accessible, and shall not penetrate watertight bulkheads. Metallic fittings must be installed where flexible fuel hose penetrates the bulkhead. 46 CFR 182.20-40(a)(2)(i) OLD T.

Approval of bowls of other than steel construction will specify if a flame shield is required. Racor fuel strainers in the lazarette do not have drip pans as per manufacturer design.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Aft bilge strainer is not 3 times the area of the suction pipe.

Electrical equipment on a vessel must be installed and maintained to: minimize accidental personnel contact with energized parts. Four dead end wires were found under the pilot house helm.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Anchor and stern light is not operational.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Fire extinguishers were not permanently affixed to the bulkheads.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Marking on fuel shutoff not present.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Retroreflective tape was missing from ring life buoy.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters. Life jacket quantity was not labeled.

A pre-engineered fixed gas fire extinguishing system must: Be approved by the Commandant; Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector; Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space; and be installed in accordance with the manufacturer's instructions. A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge; and (3) A means to reset devices used to automatically shut down ventilation systems and engines. The pre-engineered Fireboy gas fire extinguishing system for the main machinery space, was not equipped with a means to manually activate the system, or a means to automatically shut down the engines that draw intake air from within the protected space. The owner shall install a means to manually activate the system from outside the protected space, and a means to automatically secure the engines that draw intake air from within the protected space upon system activation. All installations shall be in accordance with the approved Fireboy installation manual.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years. The vessel has not undergone the required Coast Guard drydock and internal structural examination for more than two years. The vessel is limited to service as an un-inspected passenger vessel carrying 6 or less passengers with at least one for hire, or a recreational vessel, until it undergoes the required drydock and internal structural examination, or is granted an drydock examination extension by the Coast Guard.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety Radio Certificate was missing. A new FCC Safety Radio Certificate must be obtained and placed onboard the vessel.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. No record of monthly maintenance being performed on portable fire extinguishers. Provide evidence that contains a record of all monthly maintenance being performed on portable fire extinguishers.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. The Port fuel tank supply line shutoff valve was severally corroded and not functional. Install new fuel shutoff valve on the port fuel tank supply line.
