Vessel could not move "Dead Slow Ahead" when instructed by Pilot. Suspect that No.2 air starting pipe, on the MDE, came out of the union and was bent. This is based on preliminary vessel determination. Requesting certifying issuing authority for clarification and determination of cause.

Resolved 21 February 2017
Resolution: Main Engine failure due to the MDE cylinder Unit No. 2 starting air valve pipe union detaching during maneuvering. Pipe was reattached and tightened. No. 1, 3, 4 and 5 were also inspected and tightened. Repair satisfactory to the attending surveyor.
