46 CFR 185.510(a) The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing actions to be taken in the event of fire, heavy weather, or man overboard conditions.

46CFR180.71 An adult life jacket must be provided for each person carried on board a vessel. 02 life jackets were missing.

46 CFR 180.70(b)(1) Each life ring on a vessel must be approved in accordance with 160.050 in Subchapter Q of this chapter. Ring buoy’s grab line supports were separated.

46 CFR 182.455(b)(4) The shutoff valve at the tank must be manually operated from outside the compartment in which the valve is located, during a manual test of the STBD side fuel tank shutoff valve, valve handle at control station was broken off and could not be pulled with out pliers.

46 CFR § 185.730 During the inspection of the vessel's primary lifesaving appliances, a life float was found to have visible cracks in its outer shell. Replace or properly repair the cracked life float IAW manufactures instructions.

46CFR180.71(b) In addition, a number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. o1 child lifejacket was torn.

46CFR185.520(f) Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Log book was lost.

Based on inspection findings, the vessel currently lacks the necessary equipment or material condition to safely operate on its fully certificated route. Specifically, the vessel was found to have an unserviceable life float required for Limited Coastwise route. Consequently, the vessel does not meet the requirements for its current route, and an operational restriction must be applied for the safety of passengers and crew. The vessel's permitted route is temporarily reduced to Lakes Bays, and sounds. The vessel must not operate beyond this restricted route. The Certificate of Inspection (COI) must be amended by the OCMI to reflect this change, OR the underlying deficiency must be fully rectified prior to operating on the original certificated route.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S Coast Pilot [name withheld] list, Tide Tables, and Current Tables. Publications missing.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Engine room and fwd compartment indicators are inoperable.

On a vessel of at least 26’ in length a visual and audible alarm must be provided at the operating station to indicate a high water level. Engine room and fwd compartment visual indicators are inoperable.

Monthly portable fire extinguisher inspections required by NFPA 10 may be conducted by the owner, operator, person in charge or a designated member of the crew. No record of monthly inspections could be produced. Inspection conducted and logged for today. Corrected on the spot.

The master of the vessel shall ensure that the EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPIRB test were not being conducted or logged. Test conducted and logged for today. Corrected on the spot.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. Proof of enrollment in consortium is expired, alcohol testing devices are expired, and proof of EAP supervisor training missing. 46 CFR 16.230 46 CFR 4.06-15 46 CFR 16.401

The provisions of Part III of Title III of the Communication Act require vessels carry a radiotelephone installation complying with this subpart. FCC Radiotelephony Certificate is expired. Vessel restricted to not more than 1000 feet from shore. 47 CFR 80.901

Each EPIRB must be tested monthly. The EPIRB test shall be logged or otherwise documented. Log for EPIRB tests and for drills not on board. 46 CFR 185.728 46 CFR 185.524 46 CFR 185.520

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Owner/Operator failed to make vessel available for annual inspection. Vessel must undergo an annual inspection prior to carriage of passengers.

A vessel not required to have a power driven fire pump, must have at least 03, 9.5 liter buckets, with an attached lanyard. Fire buckets are holed and wasted.

Each life ring on a vessel must be approved in accordance with 160.050 in Subchapter Q of this chapter. Ring buoy’s grab line supports were separated, retro tape was wasted.

Every vessel subject to Part III of Title II of the Communications Act must have a detailed inspection of the radio installed by an FCC-licensed technician in accordance with 80.59, once every five years. Vessel's certificate has been expired for over a year. Vessel is restricted to no more than 1000' from shore.

The master of the vessel shall ensure that the EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPIRB test were not being logged.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill shall include the following information, (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. No documented evidence of drills being conducted other than random dates written.

A vessel must carry either a 1st aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents. 1st aid kit has expired items as well as missing several items.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel always leaves port and at all times while underway. Life float is showing exterior material cracking , retro reflective tape is non functional, repair according to manufacturer's recommendations.

A fishery endorsement entitles a vessel to employment in the fisheries as defined in 67.3, subject to federal and state laws regulating the fisheries and in any other employment in which a registry endorsement is not required. Vessel does not have a fisheries endorsement.

Master shall conduct sufficient fire drills to make sure that each crew member is familiar with his/her duties, crew was not available to evaluate drill.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and internal structural exam at least once every two years. Vessel past allowable time to complete satisfactory drydock and internal structural exams. 46 CFR 176.600(c)(1)
