The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to...other hazards. The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely affected its maneuverability and posed a hazard to vessels and infrastructure along the Houston Ship Channel. Submit for review a Classification Society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full and normal maneuverability has been restored.

Resolution: Crew satisfactorily cleared forward clogged nozzles and realigned manifold spray valves to effect satisfactory test of Deck Water Spray.

The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protections of the environment. The various tasks should be defined and assigned to qualified personnel. The crew could not produce procedures for testing and flushing of deck water spray system. PSCO observed isolation valves for port and starboard manifolds to be closed during test of deck spray while ship was connected for cargo operation on a cargo waiver. 74SOLAS(14CONS)VII/13.1 REF: ISM 7 50a/c
