Each vessel vapor collection flange face must have a permanent stud projecting outward. It must be located at teh top of the flange between bolt holes, and in line with the bolt hole pattern. PSCO observed permanent stud port side forward vapor line was loose.

In spaces where penetration of oil products is possible, the surface of insulation shall be impervious to oil or oil vapors. PSCO observed main diesel engine's fuel return line lagging soaked with fuel oil. Ship's crew quickly replaced lagging.
